Job Description: The Forensic Mental Health Therapist provides high-quality, confidential, and affordable outpatient mental health services for adults at our Adult Mental Health Clinic and at the Chemung County Jail. Forensic Mental Health Therapist is responsible for clinical assessment and therapy services, including risk assessment and crisis intervention services, at the Chemung County Jail for approximately 16.5 hours per week, with the remaining hours to be completed at our outpatient Adult Mental Health Clinic. They address a wide range of emotional, behavioral, relational, environmental and/or biological issues or concerns. Our outpatient clinic and 14 offsite locations are licensed by the NY State Office of Mental Health and is National Health Service Corps (NHSC) certified. Responsibilities: Provides outpatient clinical services including assessment, screening, diagnosis, treatment planning, case management, crisis intervention and ongoing treatment; Supports safe, quality-based behavioral health care services. Respond to emergent client needs, including risk, other safety assessments, and referral for further evaluation. Engages clients from a person-centered, strength-based holistic orientation addressing medical, behavioral health, long-term/short-term care, and social supports. Collaborates effectively with agency staff and other community agencies on behalf of clients. Work in collaboration with the Chemung County Correctional Department, including the New York State Courts. Complete all required documentation, including but not limited to behavioral health screening, comprehensive behavioral health assessment, needs assessment, risk assessment and safety plan, plan of care, progress notes, Office of Mental Health forensic placement forms, and discharge summary. Schedule competency evaluations as directed by Chemung County Courts and Director of Community Services of Chemung County. Submit outcomes of evaluations to appropriate parties in a timely fashion and facilitate transfers to/from forensic psychiatric units for transition of care. Aim to reduce recidivism rates by completing comprehensive needs assessments, facilitating appropriate referrals to community agencies, and aid in the transition of care post release from the Chemung County Jail. Short term follow-up post release for continued case management support and referrals as needed Responsible to represent the agency in the community with courtesy and professionalism. Responsible to review and adhere to all policies and procedures at the agency and Chemung County Jail. Other assignments and tasks may be added at the discretion of the supervisor, Clinical Director, HR Director and/or CEO. The ideal candidate will have at least three years of clinical practice in a behavioral health setting with demonstrated experience in forensic mental health counseling. Able to work collaboratively and independently while maintaining professional boundaries and meeting deadlines/expectations. A master’s degree in social work, Mental Health Counseling, or related licensed clinical degree is a requirement.
