JOB SUMMARY:
Under the direct supervision of the Ryan White Program Manager, the Medical Case Manager will provide a range of services that link clients with primary medical care, psychosocial services, benefits and other supportive services. Provides outpatient HIV medical case management services and patient advocacy through education, counseling, advocacy, benefits assessment and enrollment, and coordination of services. Ensures patient access to essential medications, medical care, and support services.
DUTIES PERFORMED:
• Intake and initial assessment of services needed
• Development of a comprehensive, individualized action plan
• Coordination and linkage of services required to implement the plan
• Monitoring to assess the efficiency of the plan, to include home visits when necessary
• Review and reevaluation of service plans on a regular basis, not limited to, at least every 6 months.
• Deliver medical case management services in accordance with DHEC standards
• Enroll and recertify clients in SC ADAP (AIDS Drugs Assistance Program)
• Provide ongoing benefits counseling, support and information, including prevention and applicable education.
• Provide referral to, and coordination with, public or private programs, including hospitals, physicians, clergy and community agencies to ensure continuity of care in the home or an institutional setting.
• Provide timely and confidential documentation of all case management activities to include all client contacts and updating records in Provide database necessary for accountability.
• Maintains knowledge of community resources and means to access services.
• Performs advocacy services as needed.
• Facilitate support groups and programs for agency clients as needed.
• Assist clients in obtaining HOPWA (Housing Opportunities For People With HIV/AIDS) services on an as needed basis
• Provide clinic and program information, emotional support, help to locate treatment information, find community resources and referral sources.
• Assist with annual Focus Group meeting and Consumer Advisory Board activities as needed.
• Attend weekly inter-disciplinary team meetings and agency staff meetings as scheduled.
• Document all attempts to link clients to care into the Provide Data System.
• Observes schedule for work, breaks and lunch.
• Must hold all patient Protected Health Information (PHI) other patient personal information and agency information in confidence.
• Actively participate in and comply with all aspects of the NHFHS Corporate Compliance Program, follow the Program Code of Conduct and obey all relevant laws, statutes, regulations and requirements applicable to Medicaid, Medicare and other State and Federal health care programs.
• Performs any other duties assigned.
• Participates in CQI, other internal committees, special project/observances or activities that promote improvements in organizational performance and/or advance the mission, goals and objective of New Horizon Family Health Services

QUALIFICATIONS:
• Bachelor's degree from an accredited School of Social Work or Bachelor's degree in a related field.
• An Associates Degree in Human Services or Health Care field and two years experience in medical care setting or High School graduate and four years work experience in medical care setting
• Minimum 1-2 years case management, social work, or equivalent health related experience
• Preferred:
• Minimum 1-2 years experience with HIV/AIDS or high-risk population preferred
OTHER REQUIREMENTS:
• Must complete DHEC Medical Case Management Educational Training Series and must pass the final examination within 18 months of employment.
• Knowledge of HIV/AIDS issues and familiarity and comfort working with HIV positive persons.
• Must demonstrate the ability to exercise sound judgment and discretion.
• Must be able to understand and follow written policies and procedures.
• Must demonstrate the ability to communicate effectively both orally and written.
• Must have excellent customer service skills.
• Must be able to establish and maintain effective working relationships with co-workers.
• Ability to use office equipment, computer software including MS Office and practice management software as trained.
• Ability to be an effective client advocate. Ability to help clients understand and prioritize family needs.
• Must have understanding of the goals and mission of community health outreach and the community care system.
