NEW HORIZON FAMILY HEALTH SERVICES POSITION DESCRIPTION JOB TITLE: Outreach & Enrollment Specialist DEPARTMENT: Administration SUPERVISOR: Care Management Supervisor **IMPORTANT NOTE** THIS DOCUMENT DOES NOT CREATE AN EXPRESS OR IMPLIED CONTRACT OF EMPLOYMENT JOB SUMMARY: The Outreach & Enrollment Specialist (OES) conducts a variety of outreach and enrollment activities designed to identify eligible individuals and provide assistance with accessing affordable health insurance and other essential resources. The OES represents NHFHS at various community forums and events to increase awareness and promote support for comprehensive, sustained community programs designed and targeted to meet the primary health care needs of the medically underserved. The OES develops new collaborative efforts and partnerships to advance the goals of NHFHS. DUTIES PERFORMED: Outreach and Partnership Development: Provides information regarding the Affordable Care Act and Health Insurance Marketplace to NHFHS’ staff and patients. Identifies opportunities for NHFHS and patients to participate in collaborations, partnership and coalitions related to the implementation of the Affordable Care Act and Health Insurance Marketplace. Provides training to targeted audiences regarding health care programs in the community to increase awareness of program eligibility requirements and services covered. Prepares and submits a monthly progress report. Actively participates in a variety of coalitions and interest groups to develop relationships with groups involved in Outreach and Enrollment. Demonstrates and maintains expertise in eligibility and enrollment rules and procedures, the range of qualified health plan options and insurance affordability programs, the needs of underserved and vulnerable populations, and privacy and security standards. Conducts public education activities to raise awareness about coverage options available under Medicaid and the Health Insurance Marketplace. Helps individuals understand and access affordable health insurance coverage options. Provides information and assistance in a fair, accurate, and impartial manner that is culturally and linguistically appropriate to diverse communities and accessible to individuals with disabilities. Provide referrals to any applicable office of health insurance consumer assistance or ombudsman to address consumer grievances, complaints, or questions about their health plan, coverage, or a determination. Successfully completes and complies with all required and applicable federal and/or state consumer assistance training. Participates in all training required by the company and/or provided by partner organizations. Demonstrates capacity to conduct “in reach” with current health center uninsured patients and “outreach” to members of the public regarding access to health coverage in their approved service areas. General Responsibilities Adheres to approved schedule for work, breaks and lunch. Complete and document a minimum of 100 contacts each month. Provide an activity report monthly within 5 working days of the end of the month. Must hold all patient Protected Health Information (PHI) and other patient personal information and agency information in confidence, in accordance with the Employee Confidentiality Statement. Participate in CQI, other internal committees, special projects/observances or activities that promote improvements in organizational performance and/or advance the mission, goals and objectives of New Horizon Family Health Services. Actively participate in and comply with all aspects of the NHFHS Corporate Compliance Program, follow the Program Code of Conduct and obey all relevant laws, statutes, regulations and requirements applicable to Medicaid, Medicare and other State and Federal healthcare programs. Perform any other duties as assigned by the supervisor or Chief Executive Officer. QUALIFICATIONS : Education A Bachelor’s or Associate’s Degree in Social Work, or related human services field preferred. Experience A minimum of 1 year experience with accessing community resources, agency networking, community outreach, patient education, or client advocacy is required. OTHER REQUIREMENTS: 1. Excellent communication skills in both one-on-one and group settings. 2. A strong motivation to help others and ability to work with diverse populations. 3. Demonstrates the ability to develop effective working relationships with a variety of individuals to gain support and interest in the program's objectives. 4. Demonstrates ability to apply knowledge of a variety of federal, state and local resources, programs and services involved with Outreach and Enrollment initiatives. 5. Demonstrates the ability to use MS Office, Google, the internet, e-mail, spreadsheets and presentations. 6. Demonstrates the ability to work independently with little direct supervision in the efficient and thorough completion of tasks. 7. Demonstrates the ability to prioritize multiple tasks, assignments and projects to meet established quality and time requirements. 8. Demonstrates the ability to work in a team environment.
