Intake coordinator Patients First Home Care is looking for a Weekend Intake Coordinator. The schedule for this position is: Thursday to Sunday working Hybrid, with two days IN OFFICE, typical Thursday and Friday. If you are interesting and live in the Sarasota Area Apply today! Assist in the coordination of patient care, under the direction of the Clinical Manager, Branch director. Answer phones, receive all intake information for paraprofessional cases, and enter the appropriate information, in the computer. Complete intake forms and appropriately route them, for admission approval. Assist in the scheduling of appropriately skilled person to fill the case, input scheduling information on the computer, and continue to assist in the coordination of the scheduling. Schedule replacement workers, as needed, and input scheduling information. Take telephone referral information. Perform other duties, as directed. Enter patient information into computer, check Medicare eligibility. Communicate with Case Managers, families, physician offices or facilities.
