Youth Intervention Services (YIS) Counselor
Description
PURPOSE:
To provide community counseling services to include outreach, intake, assessment, interventions and referral services for both individuals and groups.
Key Responsibilities
• Conduct intake assessment, psycho/social assessment, suicide risk assessment.
• Open an average of six (6) new clients a month.
• Provide crisis counseling and referral linkage to individuals and families.
• Provide school based and/or “in-home" crisis counseling as needed.
• Develop service plans in collaboration with clients and enter into Case worthy.
• Address problems identified on the service plan with clients and families.
• Responsible for client files and accurately recording sessions, meetings, phone calls and other pertinent information related to the client and family through progress notes and other documentation.
• Provide group counseling activities and social skill instructions.
• Participate in community outreach activities and document all outreach on appropriate form and submit back-up documentation for outreach to Operations for upload in Case worthy.
• Participate in professional development activities, association meetings and trainings, as required.
• Receives case and file review supervision from the Program Manager.
• Participate in case staffing process and truancy court when needed.

Pay range- $48,000.00- $51,000.00
Requirements
LIST OTHER MINIMUM REQUIREMENTS:
Possess valid Florida Driver’s License in good standing and be insurable by the agency’s current insurance carrier. Must meet state criminal background check requirements.
Education / Experience
Master's degree in social work or a related human service field, or bachelor's degree in social work or a related human service field and two years direct experience in the delivery of services to families and children.
