GENERAL STATEMENT OF DUTIES: The FST Wraparound Specialist will work with an identified child (age 0-12) presenting with severe emotional, behavioral, or mental health difficulties and their families where the child/youth is in, or at risk for, out-of-home, institutional, or restrictive placements, and/or involved in multiple child and family-serving systems. The wraparound specialists will utilize High Fidelity Wraparound (HFW) to implement a team-based, and highly individualized planning process that assist families in reaching their goals. The wraparound specialists will develop an individualized plan of support based on the needs of each family. Wraparound Specialists will be required to follow agency guidelines and requirements for client contacts, assessments, deadlines, documentation, and training Qualification/Experience Minimum of a Bachelor’s degree from an accredited university in the area of psychology, social work, or related human services field. Minimum of two (2) years of experience required in assessment, individualized care planning, and family-focused case management or mental health counseling. Strong foundation in providing trauma informed care, with a focus on the whole family system. Must have a flexible schedule to accommodate families’ needs. PRINCIPLE DUTIES The Family Support Team (FST) model provides family-centered services that combine traditional services with warm handoffs, exploring natural supports, and developing family protective capacities. The FST team is trauma-informed, culturally-aware, and requires experience taking a multigenerational approach The FST team works collaboratively with families to explore strengths, challenges, and identify both clinical and non-clinical supports needed. This can include coaching parents in parenting strategies and/or balancing everyday stressors like work, legal concerns, finances, and healthcare. FST team will receive training in High-Fidelity Wraparound and will be expected to follow the model to fidelity. An initial needs assessment and a plan of care are to be completed at the initiation of services as well as progress monitoring documentation throughout services. FST team will be required to follow agency procedures and expectations for documentation and quality assurance as well as to adhere to agency best practice standards and expectations. FST team be responsible for collaborating with other systems of care and community partners to ensure families access the resources they need. Abide by the Children and Families Incident Reporting Procedure (Supplement I to HRSR No. 215-6) and agency Incident Reporting Procedures. Maintain appropriate documentation in accordance with administrative regulations and according to agency policies/procedures. Ensure aftercare planning practices for all families, as well as timely and appropriate case closure according to agency policies/procedures. Attend and participate in professional development training as required by and developed by the agency. Create, strengthen and maintain positive, supportive working relationships between the child, parents or legal guardian, and all professionals involved. Uphold public trust and seek high levels of training, education, and supervision in the ethical application of counseling practices. Maintain health records in accordance with Medicaid and/or COA standards. Attend and participate in programmatic meetings as scheduled. Other duties as assigned. Ability to complete assigned paperwork in compliance with standards, including time sheets, mileage logs, leave requests, client records, client appointments and management information system. Ability to provide quality services to clients and families as evidenced by assessment of services and the quality improvement system. Attend staff development training for professional growth. Ability to provide complete documentation for services rendered within established time frames, including daily log, progress notes, assessments and case plans and other required documentation. Understanding that work will take place in the family home and/or community. Ability to work a flexible schedule in order to meet the families’ needs consistent with the practice model and program design. Understand the role in which cultural identity plays in motivating human behavior. Identify personal and institutionalized bias or discrimination. Ability to use clinical and/or other skills to carry out the program as designed. Valid Florida Driver License and full-time access to a reliable vehicle with appropriate insurance coverage.
