Summary: Provides nursing care assigned by and under the direction of a supervising registered nurse. Essential Duties and Responsibilities: (Note: Other Duties May Be Assigned) The Licensed Practical Nurse, LPN, provides nursing care as established on the physician’s plan of care and home health certification. The LPN shall provide nursing care always under the supervision of a Registered Nurse. The LPN will prepare and record clinical notes for the clinical record. LPN's provide care to the patient including the administration of treatments and medications. Report any changes in the patient’s condition to the registered nurse or to the physician with the reports documented in the clinical record. Participates in all required case conferences, utilization reviews, orientations, in-services and continuing education meetings. Development and evaluation of the plan of care in partnership with the patient; representative (if any), and caregiver(s). The LPN provides services that are ordered by the physicians as indicated in the plan of care. Preparing clinical notes. Participation in the HHA’s QAPI program. Compliance with the Agency’s Code of Conduct. Patient, caregiver and family counseling. Patient and caregiver education. The LPN will communicate with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care.'
