Job Summary
As a Patient Care Specialist, you will play a key role in providing exceptional care to patients in a behavioral health setting. You will collaborate with healthcare professionals to support the overall well-being of our patients and ensure they receive the highest standard of care.
This position is for 9am-5pm shift or 2pm-10pm shift with rotating on-call hours.
Essential Functions
• Patient Support: Assist patients with intake procedures, gather necessary information, and ensure a smooth transition into care services. Offer emotional support to patients and families.
• Coordination of Care: Collaborate with clinical staff, case managers, and other healthcare providers to ensure comprehensive care plans are followed. Coordinate follow-up appointments, referrals, and communication between providers and patients.
• Patient Education: Provide information on treatment plans, medications, and behavioral health services. Educate patients on managing their health conditions and treatment adherence.
• Administrative Duties: Handle phone calls, schedule appointments, and manage patient records. Ensure proper documentation of patient interactions, progress, and updates in electronic health records (EHR).
• Crisis Intervention: Assist patients in crisis situations by providing immediate support and contacting appropriate staff or emergency services as necessary.
• Compliance & Confidentiality: Ensure all patient information is handled in compliance with HIPAA regulations and RiverValley Behavioral Health policies. Maintain strict confidentiality and professionalism.
• Other duties as assigned.
Education And Experience
• High school diploma or equivalent required; associate or bachelor’s degree in a healthcare-related field preferred.
• Experience in a healthcare, mental health, or behavioral health setting is a plus.
• Strong interpersonal skills, with the ability to communicate effectively and compassionately with patients, families, and healthcare professionals.
• Ability to multitask, manage time efficiently, and handle stressful situations.
• Knowledge of behavioral health and mental health conditions, and familiarity with medical terminology.
• Proficiency with electronic health records (EHR) systems and general office software.
