Case Manager - JMHC
Position Summary
This position will be responsible for case management, service coordination, and skills building activities including the implementation and expansion of Athens-Clarke County’s Justice and Mental Health Collaboration (JMHC) Program. Specific activities will center around coordination of services for individuals involved in pre-release planning from the Athens-Clarke County Jail and post release coordination of services, individuals identified as high risk for detention, and individuals in recent crisis served through the Mental Health Co-Responder Program.
Services will include direct client support, skills building, case management, service linkage, and client advocacy to obtain mainstream services as appropriate. Additional responsibilities will include managing and coordinating services through an established shared information network.
Job Responsibilities & Performance Standards
• Assist justice system-involved individuals with Mental Illness and/or Co-Occurring Substance Abuse Disorders in locating and utilizing community resources including legal, medical, financial assistance, and other referral services
• Providing liaison services between the court system and justice system-involved individuals with Mental Illness and/or Co-Occurring Substance Abuse Disorders.
• Incorporates outreach and engagement activities to build rapport and trust among the target population
• Complement and coordinate service referral and intake processes with ACC Police Department’s Co-Responder teams
• Coordinate services among justice and mental health collaborative partners and mainstream service organizations
• Develop and manage a specialized caseload of high risk / need individuals involved in the local justice system
• Attends required trainings and implements evidence-based practices specific to individuals assessed with high criminogenic need
• Establish protocols for referral and service coordination among community partners, local law enforcement, courts officials, legal advocates, community supervision, local faith service organizations, and other community partners
• Maintenance and monitoring of care plans and communication with care teams
• Ongoing review of client progress and service note updates to document client progress toward stability
• Maintains contact with other social service agencies and health care providers involved with clients to provide information and obtain feedback on client's overall progress. May attend treatment team or other collaborative team meetings as needed.
• Coordinates entry of data into shared information network as appropriate, for individuals on active caseload
• Maintains program statistics for purposes of evaluation and research
• Works collaboratively and effectively with a diverse community team
• Establishes relationships among members of the target population and representatives of local community service organizations
• Effectively organizes time and responsibilities
• Complies with regulations on confidentiality of information, as specified by established policies
• Other duties as assigned by Supervisor
Minimum Qualifications
Bachelor’s Degree from an accredited educational institution in Psychology, Social Work, Community Counseling, or related field AND at least 1 year of related experience related to: training of mental health, working with justice-involved individuals, case management, and/or community/in-home based services. Must have a valid Georgia driver’s license (or ability to obtain a Georgia driver’s license) and ability to pass a criminal background check.
Preferred Qualifications
Master’s Degree from an accredited educational institution in Psychology, Social Work, Community Counseling, or related field AND at least 2 years of related experience related to: training of mental health, working with justice-involved individuals, case management, or community/in-home based services.
