Job Summary
The Comm Health Partnership Coordinator is responsible for playing an active role in deploying quality methodology and tools, facilitating improvement, assuring regulatory compliance and collaborating with SMHCS teams to address identified disparities and improve systems to positively affect patient outcomes.
Required Qualifications
• Require bachelor's degree in health, social, education, quality or related field. An associate degree or high school diploma and additional relevant experience can be considered in lieu of the bachelor's degree requirement on a year-for-year basis.
• Require a minimum of three (3) years of experience working in healthcare quality improvement projects.
Preferred Qualifications
• Prefer the ability to effectively analyze health data and be an active contributor in quality improvement projects.
• Prefer the ability to establish effective working relationships with members of the health care team.
• Prefer Six Sigma certification.
• Prefer previous project management experience.
• Prefer current knowledge of hospital related accreditation and regulatory issues particularly related to health equity.
