Autumn House-Case Manager I

A New Leaf's Autumn House is a 24-hour crisis program for victims of domestic violence. Autumn House serves single individuals as well as adults with children who require temporary shelter and support as a result of violence in the home. Services include Crisis hotline, Information, referral assistance and resources, Basic needs assistance, Domestic violence education and safety planning, Empowerment through self-awareness classes, Case Management, Goal setting, and Advocacy and GED preparation, Employment Assistance, Financial Literacy.
Hourly Pay Rate : $20.00
Schedule : Sunday-Wednesday 10:30am-8:30pm
Location : Mesa, AZ
Education
• High School or GED required.
• Associate or Bachelor’s Degree, preferred.
Experience
• Experience working with domestic violence, homeless or economically disadvantaged population required.
• Knowledge of community resources.
• Knowledge of and/or experience with trauma informed practices required.
• Bilingual verbally in Spanish, preferred.
Responsibilities
Responsibilities will include but not limited to:
• Role model appropriate behaviors to callers/clients in the shelter, at all times, utilizing trauma informed principles and practices.
• Provide case management, crisis intervention, domestic violence education, and resources and referral services to callers on crisis calls and to clients in the shelter.
• Communicate with hotel/motel and/or transportation agencies for client placement in overflow program and convey information appropriately to internal programs.
• Be responsible for issues relating to admission and discharge, in conjunction with the programs definition and policies as relevant to program/client needs.
• Perform screenings, lethality assessments and basic needs assessments for potential clients according to procedure. Assist with the introduction of program and facility orientation to incoming clients. Assist with client intake/exit paperwork as needed.
• Co-develop 24/7 Safety/Crisis Plans with every household, depending on the safety needs and risk factors identified by the client.
• Monitor the implementation of a strength-based service plan and progress of household towards goals.
• Assist households in obtaining community services and benefits, securing appointments with outside services and agencies as needed, including identification of funding sources and linkages to other agencies/services.
• Provide referrals and/or coordinating transportation for households to community services as relevant to client need.
• Coordinate client satisfaction surveys with individual clients on a regular basis.

• Valid Driver’s License with reliable transportation, required
• Current Auto Liability Insurance, required
• 39 Month Motor Vehicle Record, required
• Be at least 21-years of age
