The Therapist uses his/her abilities to assess and provide intensive in-home/clinical out-patient family services, using a therapeutic model. The goal is to provide a full range of services to the family to prevent child/family from disrupting in a FCH or in an adoption setting. The Therapist builds on the strengths of the families to resolve issues and maintain the family. The Therapist provides coordination of all team staff members that are assigned to the case.The Therapist will provide coordination of all team staff members that are assigned to the case. The Therapist’s responsibility is to work with children/families in the DCF Reintegration/Adoption/Foster Care Homes contract to develop permanency options for the children through reunification with the family or other permanency alternatives. The Therapist’s primary objective is to provide a therapeutic milieu in which the child has the opportunity to heal, to transition through change and to grow.
• Provides intensive outpatient therapy and family services to include attendance at initial team meeting assessments, education concerning early care and development, assist in forming a family support network, home-based support services and ancillary family-focused services.
• Engages the child and family in services through various assessments, including the bio-psycho-social assessment, family conferencing (case planning), service delivery and day-to-day practices.
• Assists families in establishing a support network by utilizing family, kinship, and community resources.
• Responsible for creating maximum billable Medicaid hours and submitting necessary paperwork for Medicaid billing
• Researches and recommends possible relative and kinship placement in the event out of home placement of the child or children would occur
• Facilitates and documents therapeutic groups, including family meetings, parenting, or multi-family groups
• Supports resource families to maintain placements and help children work toward permanency
• Develops and implements treatment plan and permanency goals with children and families, ensuring that goals are reached within established timeframes
• Determines and recommends which services are needed for the child and family, including mental health and drug and alcohol services, respite care and supervised or unsupervised visitations/interactions
• Family therapy will continue to be provided by the Therapist when an out of home placement occurs
• Makes referrals to adoption contractor when appropriate, completing necessary documentation and working closely with the adoption worker to identify/select an adoptive family
• Documents all contacts with families and collaterals on agency database.
• Completes reports for court and provides court testimony as required.
• Liaison with DCF, school personnel, and other community providers.
• Must have valid driver’s license and clean driving record, and ability to do extensive driving as services are provided in the client’s home
• Ensures clients’ rights are protected, explain Saint Francis Ministries privacy policy, and provide copies of Saint Francis Ministries Notice of Privacy Practices
• Reports critical incidents to DCF verbally within 12 hours, written within 24 hours.
• Assists in monitoring the overall client satisfaction of the program.
• Meets with Supervisor weekly to review assigned cases and submits reports as required by supervisor.
• Implements Saint Francis Ministries policies and follows directives as required. Follows and adheres to all pertinent Saint Francis Ministries Standard Operating Procedures (SOP’s), rules, personnel policies, and procedures; related accreditation and licensure standards; and federal, state and local rules, statutes, regulations, and contractual terms.
• Reports unusual incidents through appropriate Risk Management, clinical and safety channels.
• Ensures clients’ rights are protected.
• Is knowledgeable of and follows all safety procedures
Requirements
• Master’s degree in Social Work, Psychology, Marriage and Family Therapy or Counseling, from an accredited college or university
• Prefer clinical certification; LCMSW, LCMFT, LCPC
• License to practice in the State of Kansas
• MAB
• Must have experience in family-based services either through educational or work experience.
• Must be 21 years of age.
• Must pass a drug screen, KBI and Child Abuse and Neglect Registry Clearance check.
• FBI Fingerprints
• Lifting requirements of 50 lbs.
• Must have valid KS driver’s license, acceptable driving record and reliable transportation
