Registered Nurse (RN) - Resident Assessment (IRF/PAI) Coordinator, FT, Day

Job Summary
Coordinates, completes, and reviews all comprehensive assessments on patients as outlined by state/federal guidelines.
Essential Functions

• Coordinates the comprehensive assessment process and the interdisciplinary plan of care.
• Completes and reviews all comprehensive assessments on patients as outlined by state/federal guidelines.
• Reviews IRF/PAI (Medicare Inpatient Rehabilitation Facility Patient Assessment Instrument) or RAI-MDS (The Resident Assessment Instrument-Minimum Data Set), for inconsistencies and accuracy.
• Participates in the rehabilitation in service program. Mentors, orients, and supports nursing personnel in the implementation of the IRF/PAI or MDS.
• Coordinates and facilitates medical coding of IRF/PAI for inconsistencies and accuracy.
• Monitors and coordinates billing procedures. Conducts audits to ensure accuracy of billing.
• Coordinates completion of initial assessment and discharge summary.
• Communicates to the Supervisor, Post-Acute Access and Monitoring or the SNF Administrator the status and flow of patient census.
• Identifies significant changes by reviewing established criteria outlined by federal guidelines and follows through on recommendations.
• Participates in Quality Assurance activities, compliance issues, and initiates corrective action based on findings.
• Performs clinical nursing tasks and direct patient care when appropriate to complete assessments and/or discharges.
• Monitors and ensures the accuracy of recording on patient's medical records. Maintains all required reports, records, statistics, etc.
• Performs other duties as assigned.
Supervisory/Management Responsibilities
• This is a non-management job that will report to a supervisor, manager, director or executive.
Minimum Requirements
• Education - Associate degree in Nursing. Bachelor's degree in Nursing preferred.
• Experience - Two (2) years direct nursing care including at least one (1) year post-acute care facility
In Lieu Of
• Incumbents entering this role after 3/5/17 will have a grace period of one (1) year from the date of hire or transfer to enroll in an accredited BSN or MSN program and four (4) years from the date of hire or transfer to obtain a BSN or MSN from an accredited program unless differing enrollment or graduation requirement dates have been established under a previously signed MOU. To remain qualified for this position, incumbents must demonstrate continued enrollment and progress in an accredited nursing program until a BSN or MSN is conferred.
• All current GHS employees in the role of Resident Assessment Coord prior to 3/5/17 are grandfathered from the educational requirements listed above.

• RN - REGISTERED NURSE - NLRN
• Certification in specialty area preferred
