Develops, implements, evaluates and revises plan of care based on physical, psychosocial, spiritual, and educational needs of the patients and family relating to wounds, ostomy and continence.Assesses patient for changes that may hinder recovery. Collaborates with staff, physicians, and other disciplines to provide quality care delivery. Maintains clinical knowledge and skills competency to facilitate patient care, relating to wounds and ostomies, in accordance with current standards of practice. Provides for a safe patient care environment.Minimum Required Qualifications:Education: Graduation from an accredited school of professional nursing.Graduate of accredited WOC Nursing Education ProgramExperience: Must have a minimum of three years recent nursing experienceLicensure, Registration, or Certification: Current RN licensure with state of South Carolina required.Board Certification in WOC NursingAchieves and maintains BCLS CertificationSuccessful completion of Connect Care Modules: Essentials IP11O; EMAR RX210 & General Nursing; Documentation IP300 with a minimum score of 84%Position Responsibilities:Essential Functions:Perform assessment of patient's clinical condition, psychosocial, and educational needs inclusive of family needs as related to wounds, ostomy and incontinence.Subsequently develops plan of care including education based on findings that comply with current nursing standards.Implement wound and ostomy plan of care.Evaluates effectiveness of interventions, patient outcomes, and revises planned care accordingly as related to wounds, ostomy and incontinenceCollaborate with staff, physicians, and other health team personnel in the delivery of interdisciplinary patient care.Participates in the quality improvement standards related to wound and ostomy care.Maintains clinical competencies including care of patient with wound, skin or ostomy by attending continuing education sessions.Maintains patient records to ensure accurate communication and continuity of patient's medical treatment and nursing care.Utilizes effective interpersonal skills to provide professional care delivery to patients of various ages and cultures.Assesses wound and ostomy patient's physical and emotional response to treatment. Assist patient/family in coping with patient's wounds, ostomy and/or skin care problems. Attends all mandatory in-services and required staff meetings. Accepts accountability for education provided pertaining to policies, procedures and general patient care information, etc. Responsible for clinical application of information provided. Provides education to clinical and medical staff regarding wound skin and ostomy care. Serves as clinical preceptor for undergraduate/ graduate students and new employees as needed. Notifies physician and appropriate personnel of acute changes in patient's condition relating to wound, skin and ostomy care.Documents all care planning and intervention in a comprehensive manner in accordance to policy. Conducts self in a professional, compassionate and competent manner as reflecting the Bon Secours Mission. Evaluates utilization, availability, cost and effectiveness of wound, ostomy, skin care products and specialty beds, Develops and maintains standards, policies and procedures regarding wound ostomy, and skin issues with annual reviews and updates. Other Functions:Adheres to St. Francis Health System rules, regulations, and polices.Performs other duties as assigned.
