A national organization in Daviess County, Kentucky is seeking their next full-time and permanent Nurse Practitioner to join their team in their outpatient primary care center ! This organization specializes in working with patients who are 55+ and would need nursing home level care but want to remain living independently at home. This all-inclusive program of care is personalized to meet the individual health and well-being needs of our participants. This care model is well known for providing the continuum of care older adult patients need, keeps them connected to the community and allows them to live independently in their own home through services such as: Adult Day Care w Activities Dental & Vision Emergency Services Home Care & End of Life Services Hospital Care Lab & X-Ray Specialty Services Medical Transportation Nursing Home Care Nutritional Services/Meals Occupational & Physical Therapy Prescriptions Primary Care Social Services Interdisciplinary Team Consists Of : RNs Occupational, Physical & Recreation Therapists Dietitians Center Manager Home Care Coordinators Personal Care Attendants Drivers PCP Social Workers Responsibilities : As the Nurse Practitioner, you’ll be responsible for providing primary care management In collaboration with a physician, conduct initial, semi-annual, and unscheduled assessments and reassessments Participate in the development and implementation of participant plans of care in collaboration with the interdisciplinary team Educate participants and families as needed regarding self-care and specific medical conditions Document care in the participant’s medical record consistent with documentation policies.
