Job Summary
Plans and implements specific treatment programs for individuals according to principles and practices of Speech/Language Pathology. Collaborates with an interdisciplinary team in the development of the patient's individualized plan of care. Provides direction and supervision to Speech/Language students and Certified Nursing Assistants in the area of patient care. An advanced level of clinical and interpersonal skills are required.
Essential Functions

• Performs appropriate initial speech therapy assessments and reassessments and designs a speech therapy plan of care within the time frames required by the Agency.Demonstrates advanced knowledge of diagnoses / conditions seen in speech therapy.Selects, performs and interprets the results of ST examinations of neurological,musculoskeletal, developmental and pulmonary systems.Accurately performs and interprets standardized evaluations as determined by theappropriateness of the patient.Incorporates appropriate objective tests and measures to facilitate outcome measurement.Uses Agency assessments and screening tools as a part of the assessment process ex).High risk for rehospitalization tool, Falls Assessment tool, nutritional screening, screeningfor abuse and neglect, oxygen safety, spiritual assessment.Identifies actual or potential problems that would impact the ST treatment plan as well asthe overall patient plan of care.Documents assessment findings, including OASIS assessment responses, accurately andin the timeframes established by CMS and Agency policy using only Agency approved terminology and abbreviations.
• In collaboration with the patient's physician, establishes the plan of care for the individual patient based on the results of the patient assessment.Reviews available documents from the referral source that may assist in the developmentof the patient plan of care - i.e. Acute care discharge summary, rehabilitation notes.Designs/redesigns the treatment plan as appropriate for the patient's developmental age,educational level, and cultural background.Designs a plan of care that is coordinated and integrated with other services thepatient is receiving.Actively encourages the patient and caregiver to participate in the development of theplan of care.Establishes specific treatment goals based on the results of the evaluation. Long-term andshort-term goals are appropriate, achievable and measurable within a specified timeframe.Documents the plan of care timely according to Agency policies.Plans for the discharge of the patient taking into consideration other disciplines, goalachievement, and patient outcomes.Based upon reassessment findings and the patient's response to treatment and goalachievement, redefines problems and modifies goals and treatments as indicated by the interdisciplinary treatment plan.
• Meets the team / department productivity standards. Complies with established standards of efficient patient scheduling to maintain appropriatelevels of productivity.
• Provides treatment according to the established frequency and duration.Demonstrates awareness and use of proper safety measures regarding the patient, othersin the home, and self.Demonstrates good working knowledge of the various Speech Therapy techniques withflexibility and creativity in the implementation of these techniques.Interacts as appropriate with others involved in the patient's care to incorporate asinterdisciplinary treatment approach for coordinated high quality care.Provides developmentally appropriate education and training to patients, families, andcaregivers regarding diagnoses, progress and long-term functional outcomes.Documents interventions in a timely manner using only Agency approved terminology andabbreviations.Provides appropriate educational materials such as exercises to the patient / caregiver asappropriate.Provides the patient and caregiver with information regarding follow-up services whichmay include a referral to other community resources.
• Evaluates the plan of care for its effectiveness and determines progress toward expectedoutcomes throughout the patient's episode of care.Consults with team members and leadership for complex situations and when problemsare unresolved. Arranges for a conference with team members, leadership, patients,caregivers, as appropriate.Prepares the patient throughout the episode for discharge and discharges whenappropriate. Obtains written notice of discharge in accordance with the MedicareConditions of Participation. Evaluates individual practice through comparison of the individual therapist's patient outcomes (caseload) with state and national benchmark data for patient outcomes.
• Supervises speech therapy students as needed. Performs supervisory visits for Certified Nursing Assistants (C N A) in accordance withMedicare Conditions of Participation.Provides feedback to students and C N As in a professional manner. Documents supervisory visits as per State and Agency policies in a timely manner.
• Attends staff meetings, team meetings and mandatory in-services, as scheduled. Completes assignments in a timely and thorough manner. Assists with care and maintenance of departmental equipment and supplies. Performs other related duties as assigned or requested. Completes mandatory training annually as assigned. Demonstrates willingness to assist and is respectful of the rights of other team members. Assumes additional team / department responsibility as assigned. Completes requirements for annual health screening by the end of birth month. Maintains established departmental policies, procedures, objectives, quality improvement programs, safety, environmental and infection control standards. Assists with the orientation and ongoing development of Agency staff members asneeded. Promotes healthcare in the community through education and assistance at communityevents. Accepts and examines performance feedback and incorporates this into their conduct.
• Performs other duties as assigned.

• This is a non-management job that will report to a supervisor, manager, director or executive.
Minimum Requirements
• Education - Master's degree from anASHA accredited program
• Experience - One (1) year experience in Speech Pathology
