Job Summary Investigates, assesses, and plans interventions to help patients cope with social, emotional, economic, and environmental problems. # Job Responsibilities Interviews patients, assesses priorities, and writes reports while managing complex caseload. Completes comprehensive assessments of patients. Assists patients/families with referrals to community agencies and provides education and resource information. Works in collaboration with the interdisciplinary team to ensure quality support of care and safety of discharge. Collaborates with leadership on complex discharge planning issues and delays in service. Provides timely and concise documentation in the medical record, the assessment, concerns, needs, and discharge plan. Keeps plans up to date as patient progresses toward discharge. Supervises and/or trains other new employees, students, or interns as needed. Qualifications Bachelor#s degree or higher in Social Work required upon hire No experience required LSW - Licensed Social Worker in the State of Kentucky required upon hire OR LCSW - Licensed Clinical Social Worker in the State of Kentucky required upon hire OR CSW - Certified Social Worker in the State of Kentucky required upon hire Skills and Attributes Requires critical thinking skill and decisive judgment. Works under minimal supervision. Must be able to work in a stressful environment and take appropriate action.
