DaVita is looking for a Licensed Social Worker (LISW) (LICSW) (LGSW) to support our patients and their families at our Mankato, Mankato Uptown and New Ulm Dialysis Clinics in Mankato & New Ulm, MN!

Here is what you can expect when you join our Village as a Clinical Social Worker (MSW):
• Coverage: Covering 3 clinics: Dialysis at Mankato (1400 Madison Ave, Mankato, MN 46001), Mankato Uptown (1802 Commerce Dr., North Mankato, MN 56003), New Ulm Dialysis (701 N Broadway, New Ulm, MN 56073).
• Monday - Friday - 8:00 AM - 5:00 PM / split shifts between clinics. At least 2 days a month at New Ulm.
• Full time schedule - 40 hours
• Training - 6-8 weeks
• Will provide group supervision 4 hours a month that is no cost to the teammate

• Help patients understand their rights and responsibilities, and guide them in managing the physical, mental, emotional and financial demands of End Stage Renal Disease.

Requirements:
• Licensed or registered in the state of practice as required by state regulations -- (LISW) (LICSW) (LGSW)
• Meets all state required regulations to practice in the dialysis setting.
• Master's degree in Social Work (MSW) required with a specialization in clinical practice
• Demonstrated knowledge of government and private insurance programs
• Basic computer skills in MS Word, Excel, PowerPoint and Outlook as well as functional proficiency with DaVita specific applications within 60 days
