Description: Lifeworks is seeking a full-time Direct Support Professional 2 to work with adults with disabilities in Mankato, surrounding community and virtually. The Direct Support Professional 2 position will support individuals through focused activities that challenge and enrich the long-range goals of the individuals served. Direct Support Professionals carry a caseload; attend intakes and annual meetings; complete annual paperwork; and coordinate with the individuals served and their support teams to ensure person centered planning and services. How you’ll be spending your time: Working closely with program team to meet the daily needs of individual served Identifying, writing, and implementing new person-centered plans and goals for the individuals. Effectively communicating and coordinating with individuals’ support teams. Organizing group activities in the community or facilities based on the interests of the members of the group. Transporting individuals out into the community for volunteering and other outings using a company vehicle. Ensuring that individuals’ health and safety is maintained by following their individual plan of care and Lifeworks’ policies and procedures. Requirements Four-year degree in human services or a related field; or a two-year degree in Human services or related field and two years of work experience; or a combination of education and experience to equal four years. Experience working with people with disabilities. Experience managing a caseload, coordinating with support teams, writing goals, and person-centered planning preferred. Access to reliable transportation for work driving purposes. Valid driver’s license and proof of auto insurance. Candidate must complete and pass a pre-employment background study; a physical and drug screen; and have access to reliable transportation for work purposes.
