Hiring Range LPN Hiring Range - $22.50 - 24.40\hour RN Hiring Range - $26.50 - 28.70\hour FLSA Status: Non-Exempt Care Coordinator General Description The Care Coordinator is responsible for providing care coordination, case management, and/or wellness visits to SFHC patients and individuals/families served. Care Coordinator Essential Duties Assist in the development and implementation of organizational care coordination, quality improvement initiatives, and community resource initiatives. Conduct comprehensive assessments that include the medical, behavioral, pharmaceutical, and social needs of the patient, identify gaps in care and barriers to attaining improved health. Based on this assessment, and in conjunction with the patient, the patient’s primary provider, and other members of the care team, create and implement a care plan that will address the identified needs, remove the barriers, and improve the health of the patient (i.e. Chronic Care Management). Coordinate care by serving as the advocate and resource for the patient, their family and their physician, building effective relationships within the community and across the continuum of care. Consult with community partners to provide support services and resources. Keep comprehensive records of clients’ progress throughout the process, including documentation of phone calls, office visits, and referrals. Measure, improve, and maintain quality outcomes (clinical, financial, and patient satisfaction) for individual patients and the population served. Complete annual wellness visits. Coordinate transitions of care for patients in the hospital or the emergency department. Maintain required records for reporting purposes including expenses, consent forms, and other grant requirements. Complete all job functions observing confidentiality and HIPAA requirements. Perform other duties as assigned. Care Coordinator Skills Required Ability to establish and maintain effective working relationships with staff, patients, and the public. Knowledge of HIPAA privacy rules and regulations. Embraces electronic health records. Excellent interpersonal, verbal, and written communication skills with the ability to positively influence others with respect and compassion. Efficient and effective time management. Ability to work independently and use good judgment. Social and cultural sensitivity appropriate to ethnically and economically diverse participant- and employee-base. Care Coordinator Education and Experience Required High school diploma; Current LPN or RN Kansas state license; Must have and maintain CPR certification; Must maintain current Kansas driver’s license, and proof of appropriate auto insurance. Care Coordinator Education and Experience Preferred At least 1 year experience in directly-related field; Bilingual Spanish/English with the ability to speak and read both languages fluently.
