Starting salary: $61,100.00 - $72,904.00 annually. Will facilitate the development, coordination, implementation, and evaluation of patient care activities associated with drugs and alcohol. Will focus on areas of clinical quality measures and other clinical outcome measures, Patient Centered Medical Home requirements, chronic disease management, and other quality related activities. Responsibilities also include meetings and outreach activities. A master's degree in social work, psychology, mental health counseling, or family therapy and license to practice clinical discipline by the state of Kentucky is required.
