JOB Under direct supervision of the Public Health Supervisor this position supports the mission of Blue Earth County Human Services by effectively and efficiently delivering essential services to families and individuals so they can achieve their potential through active community participation. The Program Coordinator – Waiver and Family Programs provides program coordination and administration, performs intake and referrals, conducts assessments, provides care coordination, performs record keeping and reporting, and engages in population health efforts. This position also responds to emergencies and participates in professional staff development. EXAMPLE OF DUTIES ESSENTIAL FUNCTIONS: Provides program coordination and administration. Schedules and coordinates program-related meetings, presentations, and outreach activities. Maintains contact with affected Human Service internal divisions, external agencies, community groups, support groups, and other related professionals. Supervises program-related activities of support staff and students. Builds effective working relationships with providers and professionals in the community and region through integrated health care models. Participates in regional and state meetings with community health care providers, including other counties. Facilitates meetings with community health care providers. Participates in meetings and on committees or networks as directed.Manages intakes and referrals. Using a client centered approach performs client intakes and assessments for new and existing clients. Screens new clients seeking information regarding their health needs. Determines client eligibility through an analysis of the information collected. Explains available services and eligibility factors to clients. Provides care coordination for clients meeting eligibility criteria. Offers information regarding other service options to clients not meeting program eligibility criteria. Opens and provides services for clients/families meeting eligibility criteria. Provides preventive health care and education. Uses a client centered approach to provide education to clients, families, and the community regarding prevention of health risks and other relevant topics. Assists families in planning and implementing measures to decrease health risks. Refers clients and families to external resources to further address or improve health. Assists with outreach activities that encourage residents to utilize available services for health promotion in the community. Provides care coordination by using a client centered approach to provide and explain health services to clients and families. Responds to the state, managed care organizations, or other partner organizations. Analyzes and interprets assessment data. Assists clients with implementing measures to decrease risk of developing health issues or managing current health issues. Documents all necessary information supporting eligibility for services provided. Provides technical assistance and consultation to staff on care coordination and health issues affecting clients and families.Develops, reviews, and evaluates education materials. Develops education materials to meet staff and client's needs. Ensures educational materials are available and distributed accordingly. Reviews materials annually or as needed to maintain accuracy and relevance of content. Evaluates the effectiveness of educational tools in use. Provides or arranges continuing education opportunities for the staff. Develops, evaluates, and displays materials promoting family health.Performs grant management including researching and preparing grant applications. Develops clear, concise, and coherent written program plans that clearly describe existing problems, unmet needs, and future programmatic remedies. Prepares and submits grant reports. Determines and coordinates evaluation methods for implementation and reports on program activities. Acts as liaison with external grant authority. Assists with development of and implementation of any needed Corrective Action Plans. Provides centralized program support through interpretation and communication of State policy changes impacting the County. Makes independent decisions on program development, structure, and implementation; advising Public Health Supervisor on information needed to meet Department needs and expectations. Develops, implements, and oversees internal and external audit processes. Coordinates case assignment to ensure programmatic and statutory requirements are met. Is responsible for development of data driven practices, quality initiatives, and quality assurance. Works directly with providers and vendors on service delivery on multiple levels. Builds proactive business relationships; including provider and staff meetings and trainings. Monitors budgets and fiscal impact of assigned programs. Provides direction and works with staff on professional development and performance improvement. Acts as liaison between Public Health Supervisor and program staff. Performs public health functions. Uses public health theories and processes to practice. Assesses health service needs of individuals and groups. Formulates plan/intervention based on data collection and assessment. Participates in preventative health care and health promotions. Participates in quality improvement and assurance processes. Participates in public health all hazards emergency response in assigned role within scope of practice. Works with individuals and communities to provide support in areas of need. Assists with planning, consulting, investigating and providing technical assistance. Assists families with planning for emergencies. Performs record keeping and reporting. Documents, develops and maintains case files and completes agency forms. Submits reports to providers and state departments as required. Ensures all relevant documents are maintained in files including letters, medical reports, test results, case plans, and case narratives. Maintains documentation in a timely fashion.Promotes a diverse, culturally competent, and respectful workplace. Interacts positively with internal and external customers. SUPPLEMENTAL INFORMATION Monday through Friday from 8:00 a.m. to 5:00 p.m. Occasional evening and weekend hours may be required.
Salary: $34.89 - $48.88 an hour
