Case Manager - Remix Clubhouse (Youth Resiliency Program)
Position Summary: The primary responsibility of this role is to engage youth, foster healthy relationships, and build skills utilizing creative approaches to improve life functioning.
Work Schedule : 11:30 AM – 8:00 PM during school days, and 9:00 AM – 5:00 PM during school breaks & summer.
Job Responsibilities & Performance Standards
• Coordinates/provides resiliency support services.
• Provides skills training groups to members; incorporating evidence based practices/curriculums.
• Provides CSI (Community Support Individual) to members on caseload or as needed.
• Documents to account for all services provided.
• Provides overall daily care for members; preparing meals, cleaning/organizing, general oversight, etc.
• Collaborates with parents to coordinate care for members.
• Works cooperatively with other community agencies which may be serving the member (school system, DFCS, etc.)
• Charts all contacts with families, community agencies, or other collaborations for the member’s care.
• Advocates for youth with mental health and behavioral challenges.
• Maintains the strictest confidentiality of all information related to the member’s care.
• Works within a team environment and with external stakeholders
• Attends staff meetings, conferences, workshops, or other meetings as required by the supervisor or agency.
• Works cooperatively and in a professional manner with all clubhouse staff and other agency personnel.
• Assists with daily administrative tasks as assigned or needed.
• Collaborates with community organizations/places of business to incorporate outings into members’ treatment.
• Provides transportation for members.
• Monitors members’ progress towards treatment goals.
Minimum Qualifications: Bachelor’s degree in a related field OR may substitute 1-2 years’ experience working with children/youth and/or in the behavioral health field.
Preferred Qualifications: Computer proficiency, previous work with electronic health record systems, and work with children/youth
