The Licensed Permanency Specialist – Assessment Writer works to strengthen children and resource families by moving children to permanency through adoption, while preserving connections to family and community.
• Conducts home visits primarily to gather information from the family for review.
• Makes sure the home study reports are concise and complete accurate.
• Determines and recommends which services are needed for the child and family and whether the family is a good match for the strengths and needs of the child.
• Assists/and or completes the updating social histories for children with a goal of adoption.
• Coordinates and schedules Best Interest Staffing meetings.
• Completes documentation for Best Interest Staffing meetings.
• Provides counseling for children and resource families regarding moves, grief, separation, and loss issues related to adoption.
• Participates in supervisory conferences and team meetings, sharing pertinent information about children/families’ progress, collateral contacts, and relationships with other agencies/communities.
• Ability to work with the court system and follow court protocols including appropriate business attire.
• Completes reports to the Court, participates in court proceedings, including testifying, and makes sure court orders are followed, as needed.
• Completes and maintains written documentation in child/family’s case record accurately and within established time frames.
• Completes on-going assessment of children.
• Assists adoptive families through the adoption process.
• Supports resource families to maintain placements and help children work toward permanency.
• Assists in completing all documentation and paperwork relative to adoption subsidy and finalization.
• Works collaboratively with case teams assigned.
• Reports critical incidents to DCF verbally within 12 hours, written within 24 hours.
• Implements Saint Francis Ministries policies and follows directives as required. Follows and adheres to all pertinent Saint Francis Ministries Standard Operating Procedures (SOP’s), rules, personnel policies, and procedures; related accreditation and licensure standards; and federal, state and local rules, statutes, regulations, and contractual terms.
• Is knowledgeable of and follows all safety procedures.
• Reports unusual incidents through appropriate Risk Management, clinical and safety channels.
• Ensures clients’ rights are protected.
• Reports critical incidents to DCF verbally within 12 hours, written within 24 hours
Requirements
• Requires Bachelor’s degree in Social Work or Addictions Counseling from an accredited college or university.
• Prefer Master’s degree in Social Work, Psychology, Addictions Counseling, Marriage and Family Therapy or Counseling from an accredited college or university.
• Licensed to practice in the State of Kansas.
• Prefer two years’ experience working with children and families.
• Must be 21 years of age.
• Must pass drug screen, MVR, KBI, Child Abuse and Neglect Central Registry Clearance check and an Adult Registry Check
• FBI Fingerprint check if relocated to Kansas in the last 5 years.
• Lifting Requirement of 50 lbs.
• Must have valid driver’s license, acceptable driving record and reliable transportation
