Job Summary Triages incoming calls from clients, assessing their needs, giving appropriate clinical disposition, and facilitating referrals to primary providers, specialists, healthcare facilities, and community resources; using nursing process in accordance with applicable scope and standard of practice and with the policies, values, and mission of the organization. # Job Responsibilities Elicits questions, concerns, and information from caller to assess symptoms, determine acuity, identify appropriate algorithm/protocol, and develop treatment plan. Provides professional advice, education, and options based on assessment, established guidelines, and clinical judgment. When appropriate, dispatches emergency response units. Provides referrals to healthcare providers and community resources when appropriate, including locations, hours, and contact information. Assists caller with scheduling appointments at referral sources when appropriate. Documents calls, responses, and referrals. Performs follow up calls to evaluate effectiveness of care and need for additional care. Pages physician accurately and timely and completes direct admit orders. Qualifications Associate#s degree or higher or Diploma in Nursing required upon hire A minimum of 2 years# relevant experience required RN - Licensed as a Registered Nurse. Eligible to practice nursing in the States of Kentucky and Indiana required upon hire Skills and Attributes Requires critical thinking skills and decisive judgment. Works under minimal supervision. Must be able to work in a stressful environment and take appropriate action.
