About the Position

As our RN Case Manager, you will coordinate early intervention and transitional case management services, including discharge planning and linkage to community-based services, to address the needs of HIV-positive patients upon release from the Maricopa County jail system. You will also coordinate and provide counseling, health literacy, and linkage to care services for HIV-positive patients within the Maricopa County jail system.

We Require
• Two years of Registered Nurse experience in a clinical setting, programmatic setting, or in an infectious disease field
• A valid unrestricted license as a Registered Nurse issued by the Arizona Board of Nursing (AZBN)
• Bachelor's degree in Nursing from an accredited institution
We Also Value
• Experience working with HIV infection, transmission, antiretroviral therapy, and treatment
• Experience working with HIV coinfections and comorbidities, such as hepatitis C, syphilis, coccidioidomycosis, pulmonary disease, hypertension, and high cholesterol
• Case management experience working with individuals with communicable diseases
Job Contributions
• Works with CHS staff to identify patients in the jail system who may qualify for transitional case management services
• Reviews inmates’ possible release status using the jail’s data systems and medical records
• Provides transitional case management services, including discharge planning and linkage to community-based HIV care and services, to patients released from jail to the community; completes an HIV nursing assessment with medical, dental, substance abuse, mental health, and social histories to facilitate linkage to appropriate referrals in the community upon discharge; works with patients to complete an individualized discharge plan; and initiates referrals to community outreach and case management agencies
• Provides transitional case management services including, but not limited to, providing counseling for incarcerated HIV patients about management of a chronic condition; and providing health maintenance information and treatment adherence counseling, including nutrition, monitoring of lab results, medication regimen and liver health.
• Reviews and monitors databases to identify HIV positive patients, coordinates care and collects data
• Documents case management, health education and care coordination in medical records
• Coordinates with CHS medical providers to round on HIV positive patients during clinic hours
• Maintains documents and CAREWare information to facilitate referrals and program audits in compliance with HIPAA, Ryan White, and Correctional Health Services requirements
Working Conditions
• Work is performed primarily in a correctional setting with some office time required
• Travel by car to multiple work sites may be required
• Valid Arizona driver's license is required upon hire
• Must be able to participate in emergency response training as scheduled and be available to respond as directed in the event of a public health emergency
• Within one month of hire, all employees will be required to provide evidence of immunity or receive immunizations for vaccine-preventable illnesses for any position within the Maricopa County Department of Public Health; required immunizations include MMR (measles, mumps, rubella), Tdap (whooping cough and diphtheria), Varicella (chickenpox), and annual Influenza; some positions require a Hepatitis B vaccine and/or a Tuberculosis test
