Position Summary Utilizing professional nursing skills and knowledge, assesses health status and psychosocial needs of individuals and families in homes, clinics and other sites, and assists clients to achieve improved pregnancy outcomes and promote optimal maternal and child health. The primary components include: prenatal and postpartum outreach, assessment and follow-up visits, developing plans of care for mother and baby, support for families experiencing pregnancy and infant loss, skilled nursing care and delegated medical functions, monitoring of health status, identifying health problems and making referrals for appropriate care, coordinating and facilitating access to services, support and advocacy for individuals and families.
Serve as a Public Health resource in areas of maternal and child health for Human Services, schools, other health care providers and professionals and the general public.  Performs counseling, information and referral, health history, and assessments.   May serve on the Child Protection Team.  Provides Child Passenger Safety education and car seat installment checks as a Certified Car Seat Technician.  May be responsible for providing routine foot care to those 55 years of age and older or under 55 with a medical condition and/or disability.  Provides community assessment, bio-medical disaster preparedness, interagency collaboration, and disease prevention and control activities, with very limited or no direct supervision, but in accordance with the Minnesota Nurse Practice Act. Documents all care and activities in appropriate documentation systems.
Essential Duties and Responsibilities • Assesses and evaluates the health status of women, infants and children, families in homes, clinics and other sites in the community.
• Based on assessment, provides nursing care, observation, re-assessment, health counseling and teaching, within the Nurse Practice Act and agency policies, developing plans of care and coordinating services for individuals and families with health risks and concerns.
• Develops and maintains ongoing communication network with community groups and referral resources to provide information on available services and promote appropriate referrals.
• Participates in the development of health education programs in the community and presents programs in a variety of settings relating to maternal and child health issues, as needed.
• Carefully documents all nursing care, teaching, assessments, interventions, and supervision in the client medical record, through appropriate computer records and written reports.
• Prepares necessary reports for daily activities, time and expenses accurately and timely.
• Participates in professional development to maintain and increase knowledge and skills consistent with professional standards of nursing practice to meet legal requirements for registered nurse licensure.
• Organizes and participates in community screening activities as needed and participates in community assessment and evaluation activities.
• Participates in quality improvement and performance management activities.
• Other duties as assigned or apparent.
Qualifications • Graduate of National League for Nursing Accredited 4-year college or university with a Bachelor of Science or Arts Degree in Nursing with a minimum of 4 years' experience preferred as a current Licensed Registered Nurse or Certified Public Health Nurse; OR
• Graduate of a 2-year Associate Nursing Degree program with a minimum of 8 years' clinical experience preferred as a Registered Nurse or Certified Public Health Nurse.
• Experience with at-risk, problem pregnancies and at-risk families preferred.
• Independent decision-making and assessment experience.
Knowledge, Skills and Abilities RN position is Grade 9 with an entry hourly wage range of $29.54 to $33.12, DOQ.
PHN position is Grade 10 with an entry hourly wage range of $31.02 to $34.78, DOQ.
