Hill Valley is looking for a dynamic  regional case manager to oversee the Managed Care stays of residents in multiple skilled nursing facilities in Virginia.
We want a regional team member who is a strong patient advocate. We want a nurse with MDS experience who understands the work flow associated with case management.  We are looking for a self-starter and a quick learner who can work independently without supervision.   We want a great communicator,  leader,  and a strong member for our regional facilities.
This position will work with facilities in their area to respond to with insurance case managers and with concurrent reviews of managed care stays.  This is a remote position with only and requires Point Click Care knowledge and a strong computer skills with Microsoft Office.
This position will report to the Director of Case Management in supporting centers in need.
A current RN nursing license to practice in the state or a compact license is required.  A working knowledge of Medicare and Managed Care regulations required. A PASSION for long term care and resident advocacy  is also required and the willingless to work with teams of interdisciplinary professionals.
