Program summary: The Opioid Use Disorder Program allows Advantage Behavioral Health Systems, which provides services in a 10-county area in Northeast, Georgia, to help clients battling opioid addiction in four distinct areas, including crisis support/detox/medication assisted treatment (MAT), intensive residential and outpatient treatment, peer-led recovery support services and community outreach and education.
Position summary: The Medication Assisted Treatment (MAT) Coordinator is a case manager with the responsibility of managing all MAT referrals and intakes into the PROUD Program. The MAT Coordinator will lend assistance and services to clients through an on-going, mentoring relationship, in order to aid these clients who are seeking their own sustained recovery from alcohol and other drug abuse. The MAT Coordinator will utilize active listening, open-ended questions, will manage personal biases and be willing to share his/her lived experience with addition, treatment and recovery with clients who seek long-term recovery from addiction. The MAT Coordinator will work as a peer to build a working relationship with specific and personal knowledge of difference pathways to recovery, in order to help clients for up to 6 months following discharge. The approach will be client-centered, drawing from the strengths of each client being served. Service delivery will be personalized according to the clients needs, experience, perceived barriers and goals, creating an on-going system of social, emotional, instrumental and spiritual support with linkages to community resources.
Key Duties And Responsibilities
• Coordinate Medication Assisted Treatment with prescribers
• Assist in filtering and sorting of referrals into the program
• Interview consumers and/or family members using acceptable techniques that are appropriate for the clients level of functioning to obtain necessary information regarding medical, social, and/or emotional background needed or indicated for variations in metabolic indices
• Participate in client intake process, treatment team meetings, crisis interventions and crisis prevention planning
• Staffs complex cases with physicians for direction; consult with other staff members related to patient care
• Assist with appointments (MAT clients) and follow/document progress
• Provide education on diagnosis, medication use, side effects, treatment adherence and other topics as individualized for the patient
• Provide basic case management and linkages by making referrals for changing levels of care or to community partners
• Lead education sessions for clients, their families, other supports and allies
• Develop peer relationships with clients as they enter detox or treatment setting/program
• See clients as a whole person in the context of the persons roles, family, community
• Motivate through hope and inspiration
• Support many pathways to recovery
• Provide community education on medication assisted treatment (what it is and is not), create and lead community support/recovery groups
• Act as a treatment liaison with probation, parole, court entities, as needed
• Oversee progress in residential setting
• Follow progress in outpatient setting
• Link to resources for needs, including legal, housing, food, employment, childcare, medical, educational, transportation and family support
• Attend monthly Med P&T meetings or program meetings
• Respond to phone inquiries of patients and pharmacies
• Attend all staff meetings and complete all required Agency trainings
• May be shadowed by peers on the job
• Other duties as assigned
Minimum Qualifications: High school diploma or GED. Certified Addiction Recovery Empowerment Specialist preferred, CARES eligible
Preferred Qualifications: Bachelors degree in Social services or related field plus one year of related work experience. Exposure to persons with addiction histories and/or persons with co-occurring disorders such as Severe and Persistent Mental Illness (SPMI) along with history of addiction.
• Solid Case Management experience
• Strong communication skills, recovery language, motivational interviewing training, WRAP training
• Experience in residential settings for substance abuse
• Experience with court and legal systems
• Experience building community groups to meet the needs of diverse populations
• Ability to calm, de-escalate and provide consistent support for unstable clients
• Ability to coordinate and organize delivery of services
• Ability to interview clients and families using established techniques
• Ability to collaborate with other treatment providers to develop client treatment plans to include recovery goals and basic needs (e.g. housing, social, education, vocational, family)
• Intermediate knowledge of bio-psychosocial (BPS) aspects of behavioral health in order to fully participate in treatment team meetings and treatment delivery
• Intermediate knowledge of substance abuse treatment models to educate and relay or clarify information when indicated
• Ability to develop, implement and facilitate agency and community workshops on addiction, treatment and recovery
• Ability to assess what training is needed.
