Responsibilities include:
• Conduct primary care rooming processes and relevant medical procedures for both adult and pediatric patients.
• Follow protocols and policies for clinical procedures and equipment usage.
• Document patient care accurately and comprehensively.
• Address patient messages promptly and escalate issues as needed.
• Participate in patient-centered medical home team activities, including pre-visit planning and daily huddles.
• Support outreach campaigns to close gaps in care and promote wellness.
• Utilize population health knowledge to support value-based contracts and quality initiatives.
• Engage in post-discharge outreach for identified at-risk patient groups.
• Provide patient education and coaching for wellness and chronic disease management.
• Facilitate follow-up care and referrals as needed.
• Demonstrate age-related competencies and experience with various age groups.
• Understand recommended screenings and chronic disease management processes.
• Familiarity with patient-centered medical home principles.
What you need to succeed:
• Comfortable with blood draws and injections.
• Strong computer skills, with Epic experience preferred.
• Ability to handle phone calls professionally.
• Up-to-date flu shot.
