POSITION SUMMARY This hands-on coaching leader position directs team development and systems design for ensuring a culture of high quality, individualized care for people living in Assisted Living with Cognitive Impairment and developing Team Members into Dementia Care Specialists on the Memory Support unit. Through developing systems on team development, empowered leadership roles from on the team, and systems of accountability and engagement, this role champions creation of a safe, clean, welcoming home that empowers residents to live meaningful lives despite cognitive challenges. Assists the Assisted Living Unit Manager to direct health related services, develop and implement care plans for Memory Support Residents, and welcomes new team members to support residents in a way that is centered around their goals, preferences and desires. ESSENTIAL DUTIES AND RESPONSIBILITIES The following duties are normal for this position. This list is not to be construed as exclusive or all inclusive. Other duties may be required and assigned. · Develop and maintain professional relationships with Memory Support Staff and support them in growing as Dementia Support Specialists. · Monitors resident health status to identify significant changes and collaborates with the Assisted Living Unit Manager to address identified needs. · Direct, evaluate and supervise resident care and initiate corrective action. · Assigns duties and tasks to Nurse Aide staff on a schedule or as needed and verifies satisfactory completion. · Provides education on dementia, behavioral expressions of need , and best practices for supporting people living with Dementia and Cognitive impairment care for all Memory Support team members. · Creates systems which elicit deep, knowing between team members and residents and to advance the goals and success of those who live in the memory care AL neighborhood. · Develop a culture of anticipating the needs of residents. · Develop and implement a structure for a new Lifestyles program. The program should incorporate activities for all residents which are developmentally and age appropriate, which emphasize strengths of residents, and are customizable to meet a dynamic population . · Collaborate with the Director of Philanthropy and Engagement to develop and implement a volunteer program for those wishing to serve in Memory Support. · Help design, deliver, and evaluate systems for frequent and effective communication with Family members regarding their Resident’s status and needs. · Supervise, Assist and Teach the Floor Staff in the provision of care and support of Activities of Daily Living for Memory Support Residents. · Assist the Assisted Living Unit Manager in establishing policies and interpreting those policies according to the standards of nursing practice. · Participates in community quality assessment and improvements programs. · Maintains compliance with all personnel policies, established community policies and procedures, and Federal and State regulations and standards, including but not limited to HIPAA privacy and confidentiality laws. · Other duties as assigned by Supervisor. SUPERVISORY RESPONSIBILITIES · Supervises the Memory Support Care Team direct care personnel as assigned to assure delivery of consistently high-quality care to residents and documentation of care per state and federal requirements. QUALIFICATIONS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Educational Requirements and Experience: · Knowledge of Dementia/Cognitive Impairment care and treatment standards and best practices · Experience in culture change and desire to implement person centered/directed focused experiences · Knowledge of OBRA/Federal/State regulations for Community Residential Care Facilities. · Knowledge of risk management. · Basic Budgeting and cost management skills. · Coaching or team leadership experience preferred · Two years of supervisory experience in a Memory Care setting preferred. · Bachelor’s degree preferred in SW/ OD/ Healthcare administration/or related field. Clinical Expertise: · Knowledge of standards of care and practices applicable to the delivery of geriatric services, especially to persons living with Dementia or Cognitive Change, ensuring their dignity and safety, and promoting independence.
