Description Under the general direction of the Public Health Supervisor, the Community Health Worker Serves as a bridge between the community and the health care, government and social service systems to build trust and empower priority populations by collaborating with organizations and individuals based on established care plans and department policies, rules and procedures.
Duties/Responsibilities • Collaborating with health care providers, providing health screening in the community, completing outreach activities with community organizations, and providing individual client visits.
• Complete billable and non-billable activities and produce documentation needed for successful reimbursement of community health worker services.
• Encourages and supports community members to make concrete steps toward promoting their health and managing their chronic illnesses (e.g., diabetes, asthma, vascular disease)
• Support young families and new parents in connecting to resources that will assist them in developing a successful health outcome.
• Work in partnership with public health nurse following through with identified needs in the individual care plan of the clients.
• Work with adult and waiver case managers in delivering services needed for successful health outcomes for their clients.
• Other duties to include regular attendance of agency staff meetings and staff unit meetings; completion of assignments in a timely, organized and accurate manner; maintaining a safe environment for self and others and regular attendance is essential to the maintenance of a smooth and consistent workflow of the employee's responsibilities and the functions of the Agency as a whole.
• Attend trainings as needed to successfully perform duties.
• Comply with Minnesota Data Practices Act, HIPAA and other regulatory laws, rules and statutes.
• Other duties as assigned.
Minimum Requirements High School Diploma or Equivalent.    A valid MN Driver's license.
CHW Certification from an accredited program or must obtain within 1 year of hire.
Minimum of 1 year related experience. Experience in Microsoft Office applications such as Word, Excel, Power Point, Outlook, and Publisher.
Bi-lingual Spanish skills desirable.

• Developing client rapport and providing client services to clients.
• Operating, using and utilizing departmental applications in updating and maintaining client records and files.
• Preparing narrative reports, case files, forms and materials required by the department.
Essential knowledge and specialized subject knowledge required to perform the essential functions of the job: • Knowledge of County, department policies and procedures in areas of assignment.
• Knowledge of interviewing techniques
• Knowledge of agency programs, services, policies, procedures and organization.
• Fundamental knowledge of community resources.
• Knowledge concerning the operation and use of computer equipment, general productivity software (i.e. spreadsheets, word processing, etc.) and human service customized applications and software (e.g. MAXIS, SSIS, etc.).
• Knowledge of data privacy regulations and data retention schedules.
• Fundamental knowledge of office procedures, functions and recordkeeping and basic case management activities and requirements.
• Ability to relate to people in an appropriate manner.
