Pheasants Ridge is a premium elder community that prides itself on mindful care and providing a loving atmosphere. Today, we employ over 30 bright, caring, career-minded individuals and are seeking to add to our team. The role of this position is to ensure the health and safety of all clients are met through assessments of clients, development and implementation of appropriate care/services, monitoring of client services, and guidance of direct care team. Responsibilities Administrative Functions: Complete medical forms, reports, evaluations, charting, etc., as necessary Participate in surveys made by authorized government agencies Execute infection control plan and universal precautions Communicate with clients and their representatives regarding changes in condition, medication, and medical needs Personnel Functions: Provide guidance to direct care team related to performance of delegated nursing services Conduct skills demonstration test out with direct care team member prior to their performance of said skills with clients Conduct care team supervisory visits within 30 days and as-needed thereafter (not to exceed annually) Respond to medical questions and/or emergencies via Nurse On-Call phone Assessments: Complete initial assessment of prospective clients and develop a proposed care plan Reassess new clients no more than 14 days after initiation of services Reassess clients as-need. Not to exceed 90 days from the date of the prior assessment Monitor and maintain each client’s clinical chart and progress notes Assist in facilitating coordination of client services with other service providers (i.e. Hospitals, Hospice, Podiatry, Therapists, etc.) Medication and Supply Functions: Investigate any errors and issues and implement corrective action plan if needed Document disposal of medications per regulatory guidelines Requirements Current license to practice as a Registered Nurse in the State of Minnesota.
