Under the supervision of the Director of Pharmacy, and according to standard procedure the Pharmacy Resident is a licensed pharmacist in training who provides comprehensive pharmaceutical care through integrated drug distribution, clinical services and teaching. The resident provides services consistent with services provided by clinical staff pharmacists and clinical pharmacy specialists at UC Health. The resident training program is supported by site and the corporate department of UC Health Pharmacy Services.

Responsibilities
Patient Population:
Engages in population appropriate communication.

Involves family/guardian in the assessment, initial treatment, and continuing care of the patient.
Identifies any physicallimitations of the patient and deploys intervention when necessary.

Direct Patient Care:
Provides pharmaceutical care by assisting practitioners and/or providing direct patient care by:
Reviewing patient medication history
Establishing desired outcomes
Assessing patient demographics
Developing pharmacotherapeutic plans
Determining alternatives for achieving outcomes
Developing drug monitoring plans
Implementing new drug regimens
Monitoring patientoutcomes in concertwith medication use
Documenting medication related issues in electronic medicalrecord
Interpreting laboratory data in context of medication use
Reviewing patient medication profiles and making recommendations for drug therapy management
Monitoring pharmacokinetics of drug therapy
Monitoring and preventing potential adverse, drug-drug, and drug-food interactions
Preventing and monitoring for medication errors
Completing patient specific medication consults
Provides pharmacokinetic consultations as required, specifically addressing drugs with narrow therapeutic indices.
Discusses medication order clarifications with the prescriber, documents any changesin the electronic medical record, and informs others who may be impacted of medication order changes.
Participates in the drug therapy management during medical emergencies, responds to Code Blue and Rapid Responses, and prepares medications as needed.
Participates in patientcare rounds focusingon drug therapyissues.
Counsels patients on admission or discharge medications as necessary.
Monitors drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions.
Detects, monitors, documents, and reports adversedrug reactions (ADRs)and medication errors (MEs).
Facilitates discharge planning, including engaging Medication Access Services if needed, to ensure a smooth transition to alternate site of care or home.
Promotes the use of the formulary by converting non-formulary orders to formulary, when possible, but coordinates procurement of non-formulary drugs when necessary.
Indirect Patient Care:
Conducts one medication use evaluation and reports results to the appropriate UC Health committee.
Develop a research project plan to evaluate pharmacotherapy, implement project plan, collect data, analyze data, assess potential or future changes aimed at improving pharmacy practice, optimizing clinical care, patient safety, healthcare operations, or specific question related to patient care.
Identify and complete process improvement initiatives, including Pharmacy Morbidity, Mortality, and Improvement case conference, aimed at optimizing patient safety and operational efficiency. 
Teaching and Education:
Precepts Pharm.D. students consistent with academic credentials and clinical experience
Maintains competency in uniqueaspects of drug therapy for the patientsfor which they provide care, including factors related to patients of all age groups.
Disseminates knowledge and best practices with multidisciplinary colleagues  by providing effective medication and practice-related education 
Construct and deliver formal educational activities, such as Pharmacy Grand Rounds and Pharmacy Morbidity, Mortality, and Improvement Conference, for departmental continuing education credit
Leadership:
Participate in committee meetings and contribute to departmental planning

Qualifications
Minimum Required: Minimum Required: PharmD. Applicants to international programs are graduates or candidates for graduation from a pharmacy degree program that is a minimum of five years in duration. | Residents must obtain Ohio pharmacist licensure in the first third of the program to ensure two-thirds of the residency is completed as a licensed pharmacist based on ASHP requirements. If not licensed in Ohio by the residency start date, the candidate must obtain an Ohio Intern License. | Required: Completion of eligible PGY1 Pharmacy Residency.
