Thank you for considering a career at Bon Secours Mercy Health!Summary of Primary Function/General Purpose of PositionThis position will provide direct administrative assistance to the Advance Care Planning (ACP) program. Will interact with patients at times to assist in scheduling ACP conversations, ACP document completion and/or entering patient ACP information and documents in EHR.               Essential Job Functions Provides community based support and assistance to patients and staff to implement advance care plans as deemed necessary by the Advance Care Planning TeamAssist clinical staff in for preparing patient appointments (pre-visit planning)Act as a resource to patients and families, providing answers to frequently asked questions and suggesting educational materials using ACP Program Protocols, within scope of role, training, and at the direction of a licensed clinician.Maintain comprehensive progress notes for each patient interactionMaintains good working relationships with and knowledge of applicable ACP servicesUtilize interpersonal skills and community ACP-related resources to effectively advocate for individual in the communities servedMaintains required data and documentation and prepares required report for the Advance Care Planning TeamPerform duties with minimal supervision.  May be required to occasionally work irregular hours, attend other job related meetings, and perform other duties as assignedThis document is not an exhaustive list of all responsibilities, skills, duties, requirements, or working conditions associated with the job. Employees may be required to perform other job related duties as required by their supervisor, subject to reasonable accommodation.Patient PopulationDemonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned patient population.  Demonstrates knowledge of the principles of growth and development of the life span and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his or her age, specific needs and to provide the care needed as described in departmental policies and procedures.Adults (18-64 years).Geriatrics (65 years and older)Employment QualificationsRequired Minimum Education:HS/GEDSpecialty/Major    Preferred EducationAssociates DegreeSpecialty/MajorHealthcare Related FieldLicensing/ CertificationLicensure/Certification PreferredLPN or Medical AssistantMinimum QualificationsMinimum Years and Type of Experience1 year experience in community care, ambulatory care, or inpatient care setting.Other Knowledge, Skills and Abilities RequiredWorks well in a Team Setting. Excellent interpersonal communication skills. Strong analytical, data management and computer skills.  Experience with data entry and tracking. Highly organized and detail oriented.
