Responsibilities include:
• Float between physicians and assist with various duties such as rooming patients, triage work, in-baskets, and scheduling.
• Maximize synergy between care delivery providers to identify gaps in care and coordinate patient follow-up.
• Provide pre-visit planning for the patient panel and assist in managing care transitions.
• Act as a clinical liaison to the physician care plan and actively communicate with patients.
• Suggest process improvements and contribute to managing patient satisfaction and engagement.
• Participate in patient-centered medical home team activities, including outreach campaigns and pre-visit planning.
• Utilize knowledge of population health and value-based contracts to support wellness and chronic disease management.
• Assist in completing post-ED/post-inpatient discharge outreach and provide education and patient coaching.
What you need to succeed:
• LPN or RN license in good standing (if applicable).
• Strong computer skills, with Epic experience highly preferred.
• Comfortable answering phones and floating between locations.
• Flu shot.
