Responsibilities include:
• Provide direct patient care in a primary care office setting, maximizing synergy between care delivery providers.
• Identify gaps in care, schedule required care for patients, and follow up on referrals.
• Act as a clinical liaison to the physician care plan, actively communicating with patients and suggesting process improvements.
• Participate in pre-visit planning for the patient panel, coordinate messages through electronic portals, and assist in managing care transitions.
• Maintain accurate documentation of clinical calls and patient care, addressing patient messages in a timely manner.
• Participate as part of the patient-centered medical home team during all patient visits, completing pre-visit planning and participating in daily huddles.
• Embrace the philosophy of wellness and prevention by reminding patients of screenings and immunizations due.
• Utilize knowledge of population health and value-based contracts, participating in outreach campaigns to close gaps in care.
• Support the longitudinal care continuum of patients through post-ED/inpatient discharge outreach and education.
What you need to succeed:
• Certified or registered medical assistant with competency in clinical care, customer service communication, and teamwork.
• Proficiency in primary care procedures, relevant medical protocols, and the appropriate use of medical equipment.
• Ability to effectively communicate with patients and team members, ensuring accurate documentation and follow-up.
• Knowledge of population health principles and value-based care programs, with the ability to identify at-risk patients and provide appropriate support and education.
