Job Description Advance your career, right here! The Outpatient Dietitian provides nutrition counseling to individuals in an outpatient setting for preventative health measures and chronic disease management. Primary responsibilities include assessing and monitoring the nutritional health of outpatients, as well as, providing nutrition education for these patients and their families. Collaborate with the multidisciplinary team to facilitate optimal nutritional outcomes. Evaluate and assess nutritional status of patients. Initiate medical nutrition therapy to achieve patient goals. Establish nutrition diagnosis and develop individualized plan of care with appropriate interventions. Educates patients and families on nutrition and lifestyle changes, including behavior modification tools. Program Development: Facilitates nutrition care and program planning. Develops and updates nutrition education materials according to researched based best practice. Re-assess and monitor patients to evaluate the effectiveness of medical nutrition therapy and adjust patient’s plan of care. Evaluate progress towards goals. Communicate effectively with physicians, nursing, and the interdisciplinary team to coordinate patient care Compliance with all local, state, federal, and medical regulatory, safety, sanitation and security standards. Support performance improvement (PI) studies. Performs other duties as assigned. Qualifications MINIMUM Education, Credentials, Licenses: Bachelor’s Degree in Dietetics, Nutrition or related field Registered Dietitian with the Commission on Dietetics Registration of the Academy of Nutrition and Dietetics AND Licensed as Dietitian/ Nutritionist (prior to first day of Employment) in the state where work is being performed Specialized Knowledge: Comprehensive knowledge and application of medical nutrition therapy. Knowledge of human behavior science for effecting behavior change. Strong verbal and written communication skills. Strong interpersonal skills, problem solving, time management and decision making skills Ability to counsel and motivate others Advanced computer skills Ability to develop and implement effective nutrition education programs Demonstrates knowledge and skills necessary to provide age-specific, population specific, and diagnosis related care to patients.
