Full job description
Veracity Resourcing & Services is looking for Nurses(LPN/RN) to join our team. The LPN/RN helps to fulfill the care requirements of our patients. Under the direction of the Nurse Manager, the successful candidate will work within their entire scope of practice. The LPN/RN will be fully accountable for ensuring that the utmost in competent care and safety is consistently delivered to all residents. This opportunity awaits a motivated and ambitious professional with experience.
LPN Responsibilities:
• Provide bedside care
• Administer medications and document therapeutic effects
• Prepare and administer injections and/or enemas
• Collect samples and monitor catheters
• Provide wound care
• Help coach staff as necessary
• Ensure the nurses' notes reflect the care plan is being followed when administering nursing care of treatment
• Administer professional services such as catheterizations, tube feedings, suctioning, dressing changes, vital signs, etc.
• Able to pass Background and Drug Test
• Up to date first aid and CPR certification
• Up to date Physical and TB test
RN Responsibilities:
• Prepares patients for providers by performing assessments and completing appropriate documentation
• Identifies chronic conditions and preventive health maintenance issues to be addressed during office visit.
• Efficiently manages process issues and office flow.
• Provides patient education to improve medication compliance and management of chronic disease to improve health literacy.
• Advocates for patients to reduce barriers to care within and outside of the facility, connecting them to care where needed.
• Provides leadership to the clinical team and uses critical thinking to problem solve day to day issues in the clinic.
• Provides oversight to the care of the patient and the flow or appointments in the clinic site
Requirements:
• Gather resident information and data to be utilized by the RN in comprehensive assessments, MDS+, and RAPS
• Admit, transfer, and discharge residents according to established procedures
• Request and arrange for diagnostic and therapeutic services, as ordered by the physician, and in accordance with established procedures
• Excellent communication skills and able to work in a team environment
• Willingness to work evenings and weekends
• Exceptional staff training and customer service skills
• Compassionate and friendly demeanor
• Competent to independently perform safe nursing skills and procedures
• Able to critically think and problem solve in routine and emergency situations
• Experience supervising direct support staff
