As a Case Manager, you play a direct role in saving the lives of our patients each day by providing and promoting an environment of long-term recovery. You will partner with the Clinical Director and Executive Director to establish and maintain a therapeutic environment and routine for our residents.   Requirements: Bachelor’s degree in Social Work, Psychology, Psychiatric Rehabilitation, Sociology, Counseling, Vocational Rehabilitation, Human Services Counseling and 1 year of substance abuse experience providing direct care or 1 year of clinical experience working with individuals with co-occurring diagnoses of SUD and mental health (OR) Registered Nurse with 1 year of direct experience providing services to individuals with a diagnosis of substance abuse or 1 year of experience working with individuals with co-occurring diagnoses of SUD and mental health (OR) CSAC or CSAC-Assistant Valid/Current Driver's License Ability to travel up to 25% of the time Ability to work overtime as needed   Responsibilities: Providing case management and care coordination services for clients/patients enrolled in treatment. Responsible for coordinating continuum of care activities for assigned clients/patients and ensuring optimum utilization of resources, service delivery, and compliance with external agencies and referral sources requirements. Conducting individual and family educational sessions and engaging in various duties related to coordinating treatment and discharge activities, as needed. Coordinating, in a timely manner, issues or activities relevant to the team, and/or to the team’s relationship within the facility or treatment milieu. Adheres to organizational policies, procedures, performance improvement initiatives and maintains organizational and industry policies regarding confidentiality. Performing other duties as assigned.
