Responsibilities include:
• Providing expertise in the primary care rooming process, relevant medical procedures, and adult and pediatric patient care.
• Following protocols and policies for clinical procedures and appropriate use of medical equipment.
• Addressing patient messages in a timely manner and escalating patient issues as appropriate.
• Participating as part of the patient-centered medical home team during all patient visits.
• Assisting with outreach campaigns and tactics to close gaps in care.
• Completing pre-visit planning and participating in daily huddles with the physician and care team.
• Embracing the philosophy of wellness and prevention by reminding patients of screenings and immunizations.
• Utilizing knowledge of population health and value-based contracts.
• Participating in the longitudinal care continuum of patients through post ED/post inpatient discharge outreach.
• Updating care team through documentation and collaborating with other healthcare professionals.
• Providing education and patient coaching for wellness and chronic disease management.
• Supporting follow-up for post-hospital care, chronic disease management, or specialty referral.
What you need to succeed:
• Either medical assisting experience or completion of a formal education program.
• Comfortable with injections AND blood draws.
• Strong computer skills (Epic experience highly preferred).
• Comfortable answering phones.
• Comfortable working with kids.
• Flu shot.
