We are seeking professional and confident LPN's to join our team Full Time at Jamestowne Therapy, a consistently top-rated 5-star, 42,000-square-foot inpatient and outpatient rehab facility.
(Limited PRN opportunites also available - minimum 3x 12 hour shifts, that includes 1 weekend shift, per month. The perfect learning opportunity for ADN/BSN students!)

Pay:
• LPNs may earn up to $29.50/hour, based on date of licensure
• PLUS: $3.00/hour in shift differential for working PM shift
Shifts Available:
• Day Shift: 6:30 am - 7:00 pm
• Night Shift: 6:30 pm - 7:00 am

Responsibilities:
• Works from a team approach to meet residents’ needs.
• Assesses and reports physical, mental and emotional changes in resident’s condition.
• Plans, prioritizes and initiates resident care.
• Administers medications and treatments as ordered by the physician.
• Maintains supervision of care provided by Resident Assistants (RA) and State Tested Nursing Assistants (STNA):
• Maintains staffing ratios.
• Assigns residents to caregivers.
• Initiates and follows through with disciplinary action when necessary including completion and presentation of proper forms.
• Provides guidance to RA’s and STNA’s as needed.
• Documents pertinent information using established policies/guidelines.
• Maintains continuity of care.
• Facilitates Wellness Conferences as assigned.
• Communicates job/resident related questions/concerns, and suggestions through the established chain of command.
• Provides a safe environment.
• Reports physical, mental and emotional changes in resident’s condition to attending physician and to the resident’s responsible party.
All other duties as assigned and/or appropriate to the position.
Qualifications:
• Education - Graduate from an Ohio Board of Nursing approved nursing program or school of nursing.
• Licenses - Must have an active license in the state of Ohio with an authorization to pass medications IV certification preferred.
