Job Summary Responsible for care management, population health services, and prevention services for Owensboro Health. # Job Responsibilities Identifies and resolves issues affecting the delivery of care management services, remote patient monitoring, and prevention services within our service region. Demonstrates understanding and provides support of IT platforms and implements solutions. Participates in ongoing development of the population health model. Ensures departmental compliance with regulatory requirements. Hosts and conducts monthly calls with team members to support the implementation of population health priorities as determined in Strategy Meetings. Uses data to drive discussions around improvement opportunities and progress towards goals and objectives. Escalate concerns to Director and VP in a timely manner. Participates in the hiring, training, directing, development and evaluating of clinical staff including Care Navigators, Care Partners, and/or Community Health Workers. Monitors and adheres to annual organization operating budget. Approves or monitors expenditures, purchases and other actions to ensure compliance with budget guidelines. Participates in the development and execution of health strategies. Facilitates and leads team member training events. Fosters and maintains collaborative relationships with providers, their offices and team members across the system. Qualifications Associate#s degree or higher or Diploma in Nursing required upon hire AND Bachelor#s degree or higher in healthcare related field required upon hire OR Master#s degree or higher in healthcare related field preferred A combination of education, training and experience may be considered in lieu of degree. A minimum of 4 years# relevant experience required RN - Licensed as a Registered Nurse. Eligible to practice nursing in the State of Kentucky required upon hire Skills and Attributes Requires critical thinking skills and decisive judgment. Works under minimal supervision. Must be able to work in a stressful environment and take appropriate action.
