How You’ll Help Transform Healthcare:
Manages, facilitates, and maintains clinical initiatives/projects related to practice, business development, research and education, to include strategy, design, planning, procurement, development, implementation and evaluation. Responsible for leading and facilitating significant change intended to improve clinical efficiency and promote service line growth, expansion, and market differentiation. Acts as a liaison with other clinical departments to ensure interdisciplinary and interprofessional support and adoption of new strategic services and programs. Responsible for budget preparation and project scheduling in coordination with all stakeholders across service lines.
• Supports Carilion’s hallmarks of service excellence and quality for all job responsibilities.
• Responsible for the Diabetes Management Program across the System.
• Collaborates with other members of the healthcare team to set priorities for the Diabetes program.
• Supports the integration and application of evidence-based practice and critical thinking skills to provide optimal healthcare outcomes.
• Coordinates performance improvement plans and processes and coordinates efforts across participating departments to meet and sustain quality and address any related issues.
• Facilitates process improvements in employee education, professional practice, systems (EHR), community awareness, patient education, and overall program elements.
• Drives exceptional patient outcomes.
• Maintains knowledge in the field and is a resource to providers and clinicians enterprise-wide.
• Develops system-wide metric dashboard and routinely communicates outcomes to Senior and Executive Leaders
• Active participant in patient care for identified diabetic patient admissions (IDRs, rapid responses, patient care, and patient planning of care)
• Manages the System Diabetes Management Committee
What We Require:
Education: Bachelor of Science in Nursing or 4 years of equivalent experience required.
Experience: 4 years of relevant experience including the care of people with chronic diseases, or critical care required.
Licensure, certification, and/or registration: Must be licensed as a Registered Nurse in the Commonwealth of Virginia.
Other Minimum Qualifications: Preferred experience as RN in critical care or diabetes management   Effective communication skills:  both orally and written.   Computer skills required.  Ability to work with others within a team to ensure quality patient care.  Strong problem-solving skills (critical thinking skills)
