Summary of Job Duties: ***This is located in Batesville, AR for a full-time, on site position***
The Clinical Pharmacy Specialist works under minimal supervision. A clinical pharmacy specialist provides expertise to the department not routinely available from the pharmacy staff. The abilities of this individual may include a broad background in pharmaceutical care, specifically in clinical pharmacy practice, with knowledge applicable to both inpatient and ambulatory care settings.
The clinical pharmacy specialist provides pharmaceutical care services and clinical expertise using approved disease state management protocols in collaboration with other health professionals, managing patient drug therapy/pharmacokinetics/nutrition by consult, providing patient education, and promoting high quality, evidence-based, cost-effective medication therapy strategies through clinical recommendations. The clinical pharmacy specialist participates in multi-disciplinary settings and team rounds to optimize patient care while educating providers, residents, and students. Performs these duties in the clinical areas as assigned. Qualifications: Minimum Qualifications:
• Successful completion of a clinical pharmacy residency (PGY1) OR three (3) years of proven experience in direct patient care in a clinical setting.
• Certification as a licensed Registered Pharmacist by the State of Arkansas or eligible for licensure.
Preferred Qualifications:
• PharmD
• PG Y2 residency training AND two (2) years of proven experience in direct patient care in an inpatient or ambulatory care clinical practice setting.
• Board certification (such as BCPS or BCACP) is preferred and/or expected within 3 years of working in this pm clinical pharmacy specialist position.
The clinical pharmacy specialist will also become credentialed as a clinician to provide disease state management through approved protocols in the designated clinical practice setting(s).
Additional Information:
Responsibilities:
• Provides consultative assistance to physicians, nurses, and other clinic staff. This position will consult with physicians on appropriate therapeutic alternatives, monitoring of drug therapy, and provide drug information to professional staff upon request.
• Under protocol, initiates, adjusts, monitors, and recommends changes in drug therapy based on information obtained through patient interviews, objective monitoring parameters, physical assessments as related to drug monitoring and adverse effect management, review of medication profiles, and the patient's medical record. Documents clinical interventions in the electronic health record in a timely and professional manner.
• Participates in clinical patient rounds with medical teams as assigned while entering and verifying medication orders as needed.
• Participates in formulating professional services and policies regarding drug evaluation, selection, procurement, distribution, and safe administration practices.
• Educates patients on the effects, adverse reactions, and drug interactions pertaining to their specific drug therapy.
• Evaluates and assists in medication selection through careful consideration of financial barriers, medication insurance, and disease states to provide evidence-based therapy that maximizes patient outcomes.
• Participates in discharge planning, transition of care medication reconciliation and the drug approval process
• Monitors laboratory results that are pertinent to drug therapy management and makes recommendations when appropriate.
• Collaborates with other clinical pharmacy specialists on a daily basis, and acts as the liaison between the clinical service area and the inpatient and outpatient pharmacies as needed
• Provides staff education programs if needed to either/both pharmacy and nursing staff.
• Fills in at the clinical clerkship rotation site to precept fourth year Doctor of Pharmacy students.
• Fills in at the clinical rotation site to precept pharmacy residents that includes programs for PGY1 and/or PGY2 residents.
• Participates on department, hospital, and campus committees if needed
• May perform other duties as assigned.
