Teleios Collaborative Network is seeking a Registered Nurse for a Hospice Organization in Martinsville, Virginia. The organization is seeking a self starter that is able to serve hospice patients in around the Martinsville community. The candidate must be able to commute to each location daily, while managing multiple case loads.
Working Title May Be : Case Manager, Staff Nurse, Triage Nurse, Admissions Nurse, After Hours Nurse
Reports To: Director of a Clinical Department (DPFS, DIPU, DA, etc.) Assistant Director of Patient Services
Work Schedule : The work schedule may include day or evening shifts, weekends, holidays, and extended schedules. In extenuating circumstances staff may be asked to work more than their scheduled hours in a day/week.
Position Summary : The RN provides nursing care to patients with a life-limiting diagnosis, consistent with a physician’s order, nursing assessments and individualized plans of care.
Job Requirements:
• Graduation from an accredited nursing program.
• Current, unrestricted registered nursing license to practice nursing in North Carolina and/or Virginia.
• Must maintain CPR certification.
• Valid Driver’s License and current automobile insurance.
• 12 hours of continuing education annually.
• Work various shifts that may include days, nights, weekends, and holidays.
Job Duties and Responsibilities:
• Conduct an individualized patient assessment, prioritizing the data collection based on the patient’s immediate condition or needs and prioritize care accordingly.
• Manage the plan of care by assessing, implementing, and evaluating the services provided to each patient.
• Provide support and individualized patient/family education customized to the patient and their family.
• Provide ongoing assessment of patient, recording significant conditions and reactions, and report condition to appropriate team members.
• Collaborate with members of the interdisciplinary group to assure that the aims of continuity and comprehensiveness of care are accomplished.
• Prepare clinical and progress notes for the electronic medical record and ensure all documentation is current and consistent with established standards.
• Participates in providing bereavement support to caregivers, as requested.
• Provide direction to licensed practical nurses and certified nursing assistants, in the ongoing implementation of the care plan.
• Supervisory duties of LPNs/CNA to include visits to the patient’s home at least every 14 days, patient record reviews, conferences, ongoing communication, and collaborative care planning.
• Maintain the highest standards of professional confidentiality in matters related to the patient, family, and facility staff.
• Perform other assignments as appropriate.
Work Environment:
• Combination of remote and in-office setting, somewhat close to other people when sharing office space.
• Visit patients in their home, skilled nursing facility, assisted living facility, inpatient units, and hospitals.
