Research Nurse I/II Acute-Care Allergy Service (ACAS)
Position Title: Research Nurse, Non-Acute
Department/Unit: A cute- C are A llergy S ervice (ACAS)
Shift/Hours: Days/ 40hours/week M-F
Expected Salary Range: Minimum $31.43; Mid-point $39.59/hour; Salary determined based on years of nursing experience.
Antibiotic-associated reactions (AARs) are a poorly understood constellation of cutaneous and systemic “allergic” symptoms that bring substantial concern to patients and their families. AARs contribute to over 25% of emergency department/urgent care (ED/UC) adverse drug events with high rates of acute care re-utilization -- up to 40% of infants will re-utilize acute care resources a second or third time. Although experts in treating allergic reactions, allergy specialists have historically not evaluated patients at the time of AARs. Early allergy specialist engagement provides ideal clinical management -- longitudinal assessments of the dynamic, persistent, and “worrisome” symptoms over time and management of non-sedating antihistamines and anti-inflammatory medications. Additionally, families may be given consistent messaging about the utility of subsequent allergy testing. We created a first-in-kind allergy specialist service providing Acute-Care Allergy Service (ACAS) appointments for AARs that we theorized could reduce ED/UC utilization and re-utilization while also enhancing the likelihood of subsequent antibiotic allergy testing.
The ACAS program provides a unique opportunity to engage families in much-needed, prospective acute care research for AARs. Opportunities include:
•Comparison of ED/UC reutilization for ACAS with traditional pediatrician and/or ED/UC setting visits.
•Comparison of duration of symptoms with sedating vs non-sedating antihistamines, steroids/no steroids
•Evaluation of inflammatory cascades
•Evaluation of subsequent allergy testing- is it allergy?
Research Nurse role is to recruit patients from the ACAS to engage in new research studies- collecting blood, stool, nasopharyngeal swabs, potentially tape stripping for repository- on day 1 of AAR and after resolution (1-2 weeks later).
Some patients will be recruited from the emergency room, others from telemedicine encounters (allergy team). The repository has not yet been built and will be longitudinal assessments (at least 2 time points). Additional patients will be recruited who have confirmed allergic reactions to medications upon testing (includes antibiotics but may also include any other medication).
Ideal skill sets: knowledge of all aspects of IRB protocols, patient consents, recruitment, sample collection and storage. Will need to build and enter clinical data and questionnaires into Redcap Database. Will maintain all regulatory documents and support grant applications.
Will interact with clinicians in the pediatric allergy antibiotic testing service (PATS) and likely staff from the emergency room/ urgent care research team.
