General Definition of Work
Performs work maintaining resident caseload at four (4) sites in order to provide supportive services for employment, home and financial stability, educational goals, nutritional and mental wellness needs, and aging in place, providing needs assessments when necessary and links to community resources, planning and maintaining resource programs, trying to increase program size, and other work as apparent or assigned.
This position reports to the Director of Community Support Services.
PAY: $15.63 - $21.88 PER HOUR

· Provides case management services to residents; completes needs assessments; maintains data through Department of Housing and Urban Development Standards for Success reporting and data tracking system (TAAG); hosts trainings, resource fairs, career fairs, and other events.
· Plans and maintains programming, such as Feeding Southwest Virgina Senior Food Box (CSFP) and Soup for Seniors, and plans other training programs and projects with community partners.
· Participates in community outreach by designing, developing, and printing flyers, calendars, and newsletters.
· Completes reports and related data entries.
· Coordinates with community resources to provide referrals, discuss needs, or plan necessary events and fairs.
Minimum Qualifications
Bachelor's Degree in Human Services or related field and moderate years of experience in social service delivery for low-income youth, adults, senior citizens and/or people with disabilities, or a combination of equivalent education and experience.
Preferred Qualifications
Master’s degree in Communication, Business, Human Services, or related field and considerable years of experience in Human Services, Communication, Business, Education or similar field, or a combination of equivalent education and experience.
Special Qualifications
Valid state identification or driver’s license in the Commonwealth of Virginia.
