How You’ll Help Transform Healthcare:
Responsible for coordinating operations, strategic planning and financial performance for Carilion's value based care contracts. Works with Accountable Care Strategies team, Family Medicine, Internal Medicine and others key stakeholders within Carilion to drive higher quality, lower cost care.
• Responsible for coordinating the development and execution of ACO initiatives, as approved by senior management
• Manages outreach and collaboration with internal stakeholder to support ACO programs, including Family Medicine, Internal Medicine, Specialty Departments and other departments across Carilion.
• Responsible for managing relationships with Post Acute Care providers in the service area in order to support Carilion's value based care goals
• Works closely with Senior Vice President of Accountable Care Strategies and Senior Medical Director of Population Health to establish and execute the long-term and short-term planning for value based care programs at Carilion
• Develops presentations and materials that are used by senior management team to describe Carilion's value based care programs to internal/external stakeholders
• Responsible for contracting and managing vendors supporting Carilion's value based care programs
• Responsible for management and oversight of population health managers and data analyst within Accountable Care Strategies team.
• Coordinates with Accountable Care Strategies team and other internal stakeholders to measure and improve quality performance and risk coding across value based care agreements.
• Strategically plans for growth, staffing, services, educational opportunities and effective performance improvement in collaboration with Human Resources.
• Facilitates professional growth and development including orientation in service programs and effective performance improvement systems.
• Responsible for operational goals, plans and fiscal management including budget development, implementation and monitoring for Accountable Care Strategies.
• Responsible for owning Accountable Care Strategies work to improve collaboration between primary care and specialty departments through the ACO Specialty Council
• Monitors legislation, regulatory action and other public policy impacting Carilion's value based care programs and communicates potential impacts and mitigations strategies to senior management team
• Reviews CMS/CMMI newly released models as well as updates to determine appropriateness for and impact to Carilion.
• Works closely with Senior Director of Care Coordination to ensure care coordinators are meeting engagement and graduation targets
• Supports Senior Vice President of Accountable Care Strategies in negotiations with payers on value based care contracts
• Responsible for coordinating data analytics requests and distilling key actionable insights
What We Require:
Education: Masters degree from an accredited college in Business Administration, Public Administration, Public Health or a related field
Experience: 5-7years experience in value based care operations, consulting, project management, data analytics
Other Minimum Qualifications: Advanced leadership, planning and organizational skills as well as communication skills to manage multiple and potentially large, complex departments and to direct the successful completion of initiatives and achievement of fiscal goals. Demonstrated interpersonal and communication abilities to support and promote a respectful, inclusive culture of service excellence. Proven skills in clinical, operational and human resource management, including staffing, planning, performance management, team work and process improvement. Statistical analysis and fiscal management skills. Experience in population health, value based care and managing vendors.
