This position is for our Roanoke Hospice team, working at Carilion Roanoke Memorial Hospital with our General Inpatient patients. Three twelve hours shifts 7A-7P.
The clinical social worker provides psychosocial assessments, interviews, and/or treatments and discharge planning to patients and families to assist them in coping with hospitalization, illness, diagnosis, and/or life situation.  The social worker works collaboratively with other members of the health care team, community resources, regulatory agencies and third party payers in order to facilitate the transition of the patient through the organization while maintaining the highest quality of services possible.  The social worker will adhere to the National Association of Social Work Code of Ethics.  The clinical social worker utilizes critical thinking skills, advanced problem solving, therapeutic interventions and advanced care planning in the day to day functions.

Duties include, but are not limited to the following:
• Provide psychosocial assessments of patients and families. Formulate a comprehensive care transition plan coordinated with the interdisciplinary/transdisciplinary team.
• Meet documentation standards for quality, timeliness and confidentiality of medical records.
• Remain current with social work and health care developments and seek to increase further enhancement of job-related knowledge.
• Facilitate and provide an interdisciplinary approach to patient care.
• Provide therapeutic interventions aimed at adaptation throughout the continuum of care.
• Participate in system and departmental performance improvement initiatives.
What We Require:
• Master's Degree from a college or university accredited by the Council on Social Work Education required.
• Minimum of two years social work experience in case work or counseling in a health care or social services delivery system. Experience working with terminally ill patients or home health is preferred.  Should possess training and experience in assessing and meeting the emotional and psychological needs of terminally ill clients. Demonstrates computer proficiency with office suite software.
• Must have valid Driver's license, automobile insurance, and personal vehicle for making home visits.
• Must obtain/maintain AHA BLS-HCP.
• Must have a clear criminal records check as approved through the Virginia State Police criminal records exchange and found to be free from barrier crimes.  Effective interpersonal, communication, and team working skills required.  Must successfully complete orientation and competency validation for position.  Demonstrates advanced knowledge of social work theory and practice: proven expertise as a clinical social worker; crisis intervention skills; critical thinking and creative problem solving.  Satisfactory completion of orientation; positive interpersonal skills; oral and written communication skills; analytical skills; cultural competence; integrity; team player; courteous; ability/willingness to work as an integral member of a multi skilled team.
